Supplement 147 – Second Generation Radiotherapy {WI 2007-06-B} This was a continuation of the line-by-line reading for Public Comment. C. Schadt continued to present < sup147_42 - Reading 2014-06-26 - 3 Thur Start.doc> (posted in the mtg folder. 1525 _____________________________ Working Group Six (Base Standard) of the DICOM Standards Committee June 23-27, 2014 The reading started at “Treatment RT Radiation Set Relation Sequence” (30xx,08FA) in Table C.AA.A6-1 and ended at “Purpose of Alternate Segmented Property Type Code Sequence (30xx,134F)” in Table C.AA.B2-1. The following items have been discussed: The referencing of additional SOP Instances from within the RT Course including their approval state was reviewed again in order to discuss a diagram to illustrate the concept. Enhancements were proposed and will be worked in accordingly. The RT Prescription Module contains a two-level approach for prescriptions, but it is also possible to include prescription “orphans” that are not part of the two-level hierarchy. The question was what the intention of this is and whether any other reference would be necessary. The discussion on this topic was deferred and will be discussed at the next WG-07 meeting. The draft was annotated during the discussion and at the end of the sessions posted as sup147_42 Reading 2014-06-26 - 6 Fri End.doc. The Supplement will return to WG-06 for continued reading for Public Comment. 15. Supplement 155 –Imaging Reports using HL7 Clinical Document Architecture {WI2010-04-D} For the first session, WG-08 Co-Chair H. Solomon posted <Sup155_2014-06-20.docx> and for this review session. Using the Power Point presentation he reviewed for WG-06 the goals of the Supplement, and the document structure. WG-08 members T. Schmidt, C. Melo, L. Holliman, as well as WG-20 Co-Chair H. Koenig joined this session and discussion. H. Solomon noted that a detailed line-by-line review of the Supplement would require 6-8 hours. It was decided by WG-06 that, given the nature of this supplement, an expedited line-by-line review (about 2 hours) should be suitable to determine suitability for Public Comment with illustrating all layers of information with one example. The review continued on Wednesday with the presentation of <Sup155_2014-06-25.docx>, which was annotated during the discussion. Additional Open Issues were identified. D Clunie will check and complete the list of data elements for dose reporting. There was a discussion of the merits of using “Business Names” in Sup 155, which are defined in the context of the HL7 greenCDA specification. greenCDA was seen by some members as problematic, as it defines using an alternate XML schema (which was not the intent in Sup 155). It was suggested to not reference greenCDA, and to not use “Business Names” as the name of the organizing construct. WG-08 will take that under advisement WG-20 currently plans to distribute Sup 155 to HL7 for comment only. 1526 _____________________________ Working Group Six (Base Standard) of the DICOM Standards Committee June 23-27, 2014 There was a discussion reviewing the previously decided process, format and publication of this Supplement. It was reiterated that the primary users and audience for this Supplement are those that develop radiology reporting applications. The current draft is cast as a revision/replacement of Part 20, as suggested at the March WG-06 meeting; this needs to be confirmed during the preparation for Public Comment. Next steps: it was decided that a line-by-line review will be conducted, after all, before approval for Public Comment. To get started prior to the next WG-06 meeting a 2-hour t-con will be scheduled, in advance of the next WG-06 meeting (first week of August) to start the line-by-line reading for Public Comment. Ending Document: The document, as revised subsequent to the discussion, was saved in the meeting folder as << Sup155_2014-06-25.docx> The Supplement will return to WG-06. 16. Supplement 156 – Multi-Dimensional Presentation State {WI 2008-04-C} This was a continued reading for Public Comment. J. Whitby reviewed the comments received in the Public Comment cycle (Siemens, Philips, Toshiba, Brainlab, Hassadah University Hospitals.). The WG-11 responses have been developed and presented to WG-06; they were also posted in the ‘Sups’ directory of the meeting folder. J. Whitby of WG-11 presented the draft Supplement to WG-06: < sup156_pc_2014-06-24-1330> (posted in mtg folder.) He was supported in his presentation by J. Luszcz, A. Klingler and E. Seeberger. There was a significant discussion of the draft in both sessions. A line-by-line review reached the beginning of C.11.x8 (line 455 in the last posted draft version). WG-06 directed WG-11 to make the following changes: Add new Context Groups for View Description and View Modifier rather than specify multiple baseline CIDS Remove term ‘slicing’ in favor of using MPR consistently Replace Date and Time attributes with Datetime Clarify use of Rendered Image Reference Sequence Clarify difference between Display Collection and Sequence Collection 1527 _____________________________ Working Group Six (Base Standard) of the DICOM Standards Committee June 23-27, 2014 Remove SOP Class specific rules in volume definition Use Common Instance Reference Module to simplify references throughout Add definitions for MPR Enumerated Values Clarify use of Annotation Clipping An end-of-session snapshot of the marked-up supplement was posted as < sup156_PlanarMPR_20140626_1530.docx>. The Supplement was approved for Public Comment. 17. Supplement 169, Simplified Adult Echocardiography Structured Report [WI-2012-11-A] K. O’Donnell, Project Leader/Editor in WG-12 presented < sup169_06a-EchoSR.docx> which was posted in the meeting folder. This was a continued reading for approval for Public Comment. WG-12 members E. Canfield, J. Luszcz, and R. Knipe were assisting. K. O’Donnell emphasized that this is the first time a multicolumn TID table is used. An open issue was added to explain this table. He described the pre- and post-coordinated templates. The wall motion template is to remain in its own category. Lengthy discussion of the le TID 3QZ took place. The draft places the value of a private code in the Concept Name rather than in the value field. Added an open issue for the use of private codes for Concept Names. The annotated document was posted at the end of the session as sup169_06c-EchoSR.docx. The line-by-line reading was completed and the Supplement was approved for Public Comment with revisions decided during the review. 18. Supplement 170, Server Options for RESTful Services [WI-2013-12-C] This was a review of the Public Comments and a line-by-line review for Letter Ballot. The Editor, J. Whitby, presented the draft Supplement. WG-27 members J. Philbin and K. O;Donnell assisted. The document was posted as < sup170_ServerOptions_LB_02.docx>. The Philips comments from the Public Comment cycle were reviewed and accepted. The changes made in the draft were reviewed, a line-by-line review was completed. The Group unanimously approved the publication for Letter Ballot. 1528 _____________________________ Working Group Six (Base Standard) of the DICOM Standards Committee June 23-27, 2014 19. Supplement 171 RESTful Unified Procedure and Workflow Step (RS UPS) [WI-2013-08-B] This was a continued reading for Public Comment. The Editor, J. Whitby, presented the draft Supplement. WG-27 members J. Philbin and K. O;Donnell assisted. The document was posted as < sup171_fr_UPS-RS_06c.docx>. For today’s reading the document was renamed to < sup171_fr_UPSRS_07.docx>; J. Whitby presented the latter, WG-06 discussed it. J. Whitby made revisions and notes during the discussion. The Open Issues were reviewed: Possible complications for systems that support both REST and DIMSE services The need for a user-agent to be able to have multiple filtered subscriptions The Group discussed D. Clunie’s suggestion to take into account that Part 4 is based on Part 7, which in turn is based on Part 8; this supplement should define Part 7 behavior. Based on the discussion it was decided to include in this supplement the RESTful version of Part 7. It was noted that this supplement should mirror UPS in the current Part 4; the purpose is not to change the Current Part 4. ReferenceRequestSequence was an example for the above. All items copied from Part 4 should be referenced. J. Philbin noted that D. Clunie’s comments will be very helpful. J. Whitby started the second discussion-session presenting <sup171_fr_UPS-RS_08.docx> Additional discussions included: The need for a description of the interaction between REST and DIMSE services The conclusion that UPS REST and DIMSE services need to be more tightly aligned than other REST/DIMSE service pairs (e.g. QIDO and C-FIND) At the conclusion of the session it was decided that the Part 7 web services will not be part of this supplement; rather, may be added to the Part 18 rewrite. Further, the following additions and revisions will be made by the Editor and will be circulated prior to issuing the draft for Public Comment: Moving event reports to a new message Adding a new well known UID for file subscription Clarifying the open issues The draft with the revisions made was re-posted as < sup171_PC_change_tracking.docx>. With the foregoing conditions the Supplement was approved for Public Comments. 20. Supplement 172, Quantitative Parametric Map Storage [WI-2014-04-B] 1529 _____________________________ Working Group Six (Base Standard) of the DICOM Standards Committee June 23-27, 2014 D. Clunie is the Editor of this Supplement. He presented < sup_parametricmap_01.pdf>, which was posted in the meeting folder. This was the First Reading of the Supplement. Scope and Field of Application This Supplement defines use-cases, image IODs and SOP Classes for storage of information related to quantitative parametric maps. Parametric maps include quantitative derived images computed from acquired images, whether they be used for clinical, clinical trial or basic research purposes. DICOM objects are needed so that they can be stored in the PACS and wherever possible visualized in conjunction with images for review, and preserve patient (or research subject) identification, management, and acquisition and processing technique information. Parametric maps are encoded as images just like any other. The distinguishing characteristic of a "parametric map" is that each pixel value has a "meaning" in the real world, and the name "parametric" alludes to the meaning being the parameter of something, suchas a pharmacological model. In general, the term "parametric map" is loosely used to referred to any derived image whose pixel valueshave a quantitative meaning, as opposed to acquired images, whose pixels values may or may not have meaning (e.g., CT HU versus arbitrary MR signal intensity), or derived images whose pixel values are dimensionless. Examples of parametric maps include parameters of pharmacokinetic models, such as Ktrans of the Tofts model used in DCE-MRI regional cerebral blood volume (RCBV) or flow (RCBF) used in perfusion CT normalized values based on body size, such as SUVbw used in PET diffusion parameters such as Apparent Diffusion Coefficient (ADC) used in MR DWI Some of these applications, such as SUV and ADC, have been in widespread use already, and use ordinary modality-specific derived image IODs for encoding, with quite satisfactory (interoperable) results in most cases, and it is not the purpose of this Supplement to replace that common usage. Rather, the goals are to define: additional attributes, modules, acquisition context, code sets and scaling functions that may optionally used to extend existing image IODs a new IODs to address the need for encoding some parametric maps with floating point "pixels" the basic encoding mechanism changes needed to support floating point "pixels" Floating point "pixels" are necessary in some cases for subsequent calculations and to prevent the accumulation of rounding and quantization errors, as well as to provide a full fidelity record of the work performed. In many cases, scientific processing applications have been reluctant to sacrifice intermediate precision between processing steps, and have avoided the use of the DICOM Standard as a consequence. The IOD defined in this supplement uses the enhanced multi-frame encoding. 1530 _____________________________ Working Group Six (Base Standard) of the DICOM Standards Committee June 23-27, 2014 D. Clunie reviewed the Open and Closed Issues and reviewed the Supplement for WG-06, line-by-line. The Supplement was approved for Public Comment. D. Clunie will produce a Word-format version as well to go with the pdf. The group discussed the distribution of the document for public comments. This is a significant supplement as the scope of DICOM is being extended. . There are groups outside of the DICOM Community for which this Supplement is of interest. These stakeholders also need to be reached and Public Comments invited. As these groups are not familiar with the DIOCM process, a foreword or cover note will be needed to the draft document; D. Clunie volunteered to prepare such introductory paragraph. The additional stakeholders identified were: QIBA Groups Neurology-related Groups TBI Group 21. Supplement 173, Wide Field Ophthalmic Photography Image Storage SOP Class [WI-2013-12A] This is the First reading of this Supplement. Don Van Syckle is the Editor of this Supplement. The Supplement is proposed on behalf of DICOM WG-09. He was joined by Carl-Heinz Fleischer, A. Eslami, M. Horton, S. Virmany, and J. Hemert. The draft was posted to the meeting folder as < DICOM_supWideFieldOP_PCdraft_v6.doc> Scope and Field of Application A SOP Class to enable anatomically correct measurements on wide field ophthalmic photography 8-bit and 16 bit images. Vendors have implemented new technology which enables the acquisition of OP images using wide field fundus photography. Since the back of the eye is approximately a concave sphere, taking a very wide-Field image of it introduces large error in any attempt to measure a lesion in that image (the error is very large when using a single value for the DICOM Pixel Spacing Attribute.). Therefore, DICOM WG 9 (Ophthalmology) has determined that a new Information Object Definition (IOD) is necessary to adequately represent wide field fundus photography. The OP IOD does not address wide fields, varied pixel spacing, and proper measurement of a spherical surface or other method of projection. 1531 _____________________________ Working Group Six (Base Standard) of the DICOM Standards Committee June 23-27, 2014 Manufacturers of ophthalmic photographic imaging devices have been developing OP images (using a narrow field) for many years in DICOM (ie., these SOP Classes are widely supported by the DICOM ophthalmic community). Therefore, these wide field OP imaging SOP Classes are an extension to already existing narrow field DICOM SOP Classes. The expected use cases are described below in an informative annex that will become part of the standard. WG-06 discussed in detail the concepts in this supplement and suggested changes: Revisions decided at the first session have been incorporated and version 7 was posted prior to the second session: < sup173_v7.doc The Supplement will return for further reading, at the September WG-06 meeting. 22. Supplement 174, WADO RESTful Rendering [WI-2013-12-B] This was the first reading of the Supplement. Editor J. Philbin presented the contents of the Supplement as an updated DocBook, rather than in the customary supplement format. This was posted as <part18_v3.xml>. During the presentation, as a result of discussion by the Group, he revised the document extensively. Several open issues were added. Note: Open Issues – no consensus reached in WG-06; expecting further input from the Public Comment circulation of the draft. Closed Issues - WG-06 discussed and reached consensus on the issue; however, WG-06 will entertain and discuss further comments. 23. Supplement XXX, Multi Energy CT Imaging [WI-2013-08-A] This was an advance discussion of the concepts being developed by WG-21 to be applied in this Supplement. The Editor of the Supplement is Shlomo Gotman of Philips Healthcare. He presented the Concept Paper that WG-21 has been developing (Posted: < Dicom-Multi-Energy-Images-2014-05-22End-Of-TCon - WG21.docx>.) It is notable that two practicing medical physicists of AAPM have become actively involved in WG-21 in the development of this Supplement. Multi-Energy CT Imaging: CT multi-energy imaging techniques (including scanning, reconstruction, processing) where the scanner utilizes multiple energies from the X-Ray beam spectrum, as opposite to the conventional CT imaging, when a single (accumulated) X-Ray spectrum is used. The purpose of this discussion was to obtain guidance from WG-06 on the following topics: Are these types of images always DERIVED or are they ORIGINAL? PRIMARY or SECONDARY? 1532 _____________________________ Working Group Six (Base Standard) of the DICOM Standards Committee June 23-27, 2014 Shall we have some Series-level attribute in order to query? Material Labeling (alternative term: “Material Classification”)– Use Cases for Material Labeling: Highlighting of materials that are not well separated from others. Increasing the HU value of tendons compared to muscle based on dual energy information. Better VRT visualization Coloring of kidney stones and bone removal Coloring of gout Material-Enhanced Image Material-Suppressed Image Effective Atomic Number Image Fused images Blending (mixed) images Electron density images The Editor noted that WG-21 has scheduled a face-to-face meeting in July and will begin work on the draft supplement. The Supplement will return. 24. New Business There was no new business discussed. 25.
