The surprise question: “Would you be surprised if this patient were to die in the next year? 2. Choice/need: patient makes a choice for comfort care only, or is in special need of supportive/palliative care. 3. Specific indicators: clinical indicators for each of 3 main EOL groups (cancer, organ failure, frail elderly /dementia).
