ST PETER AND ST JAMES CHARITABLE TRUST JOB DESCRIPTION Job Title: Wellbeing Centre Staff Nurse Grade: E Reports to: Wellbeing Centre Manager Accountable to: Clinical Services Manager Qualifications: First Level Registration Minimum of two years post registration experience in the care of the elderly. ENB 931 desirable Willingness to work with both elderly residents and palliative care patients is essential. To be aware of the Protection of Vulnerable Adults East Sussex, Brighton and Hove Guidelines, NMC Code of Practice, Control and Administration of Medicines and the Health and Safety at Work Act 1974. JOB SUMMARY The post holder is responsible for the on-going assessment of care needs, development, implementation and evaluation of care plans for Wellbeing Centre patients. The post holder is involved on a daily basis with clinical ‘hands on’ care. The post holder is expected to supervise and teach new staff and assist the Centre Manager and Voluntary Services Co-ordinator with training sessions for volunteers. The Post Holder is responsible for taking charge of the Wellbeing Centre in the absence of the Centre Manager as required. MAIN DUTIES AND RESPONSIBILITIES 1 Managerial To keep up to date of currents trends in community hospice support where patients attend for specific activities To monitor the use of equipment and stores, ensuring safe and economical use. document1 To follow the procedures in respect of accidents/incidents involving patients, staff, volunteers and visitors and to complete relevant accident/incident reports promptly and accurately. To report any deficiency in equipment, furnishing and matters of hygiene within the Wellbeing centre. To work as a member of the Wellbeing Centre Team and supervise the practise of other nursing members of staff. To participate in the orientation / induction of new staff/volunteers 2 Clinical To act at all times within the scope of professional practice, as laid down by the NMC. To work with the Centre Manager to ensure that patients’ total care needs are planned and met by members of the team, following identification and assessment of physical, emotional, psychological and spiritual needs on an individual basis. To ensure that the effects of the Wellbeing Centre are evaluated and changes made in overall plans in consultation with the users of the day hospice centre. To act upon any deterioration observed in patients, mobilising medical and support services as appropriate and assisting medical staff with treatments. To maintain a welcoming and supportive environment in which patients’ needs can be met, thus ensuring the optimum quality of life for each person. To report any deficiencies in staffing to the Centre Manager and Clinical Services Manager. To ensure accurate and comprehensive nursing care records are maintained in accordance with the NMC guidelines “Standards for Records and Record Keeping”. To work closely with the current HCA regarding all aspects of patient care. To participate in ensuring patients are involved in all aspects of their care. To treat all patients with dignity, respect and understanding in accordance with the philosophy of St. Peter & St. James To be aware of Centre procedures and policies and ensure same are carried out safely in accordance with agreed policies To participate in the quality assurance strategy 3 Teaching and Professional To assist the Clinical Services Manager in identifying staff training needs and to participate in training and instruction sessions on a regular basis. To participate in the on-going development and motivation of learning amongst staff. To keep up to date through professional training and adherence to PREP requirements. To deliver a range of clinical workshops and question/answer sessions for carers and patients of the centre Be responsible for maintaining own professional development and update attending courses/study days as deemed necessary document1 MANDATORY TRAINING REQUIREMENTS Two fire instruction lectures within one calendar year. Manual handling update - annually. In house training, as appropriate, depending on rosters. ADMINISTRATION/COMMUNICATION At all times the post holder: Reports to key members of the team all relevant information about the patients’ wellbeing and day to day management issues. Liaise with Community Specialist Palliative Care Team as required re patient care. Inform GP of any changes in patient care as required,. Promotes, encourages and maintains good public relations with the local community, vis a vis the good standing and reputation of St Peter and St James. Attends regular staff meetings as required. Facilitates visits from interested groups and individuals and participates in activities organised by volunteers, friends and relatives. Demonstrates effective communication with patients and their relatives, giving advice and support as appropriate. Acts at all times to ensure the health and safety of herself/himself, other employees, patients and the general public, and co-operates with nursing management and the Care Quality Commission in ensuring that the provisions of the Health and Safety at Work Act 1974 are fully observed. Undertakes any other duties that may be reasonably requested and are appropriate to the position of Wellbeing Centre staff nurse.
