 This instrument is to be reused in every
class taught in this room. Please do not
mark on the form.
 Your responses will be tallied using a
Scantron machine. Please mark your
answers carefully.
 Enter your name where asked. Leave a
blank space between your last and first
names.

 Enter your student ID number. It MUST be a
nine-digit number. If it is not entered
correctly, your grade will not be recorded. If
your ID number begins with the letter S, enter
a 0 (zero) for the S. When done, your answer
sheet should have nine filled in bubbles
with the last column blank.
 In the space titled “PHONE NUMBER” enter
your three digit SECTION NUMBER for the
“AREA CODE”. Leave the remaining space

Rules for Molecular Compounds
1. The name of the element with the lower group
number (furthest to the left) in the periodic table is
the first word in the name (CO, carbon ….)
2. If in the same group, the one with the higher period
number (furthest down) goes first (SO2, sulfur…).

3. The second element is named with the root and the
suffix –ide (CS2, carbon disulfide).
4. Use Greek numeral prefixes to indicate the number
of atoms of each element (use in first word only if
more than one atom) (CS2, carbon disulfide, CO,
carbon monoxide, N2O2, dinitrogen dioxide)

 Complete your “safety agreement” in D2L;
 Complete the “Interests in Chemistry” survey

first lab;
 Print out a report form and bring it to lab.
 Bring at least one laptop with Excell in it
per team.
