Position Purpose
To be the first point of contact for clients and families seeking support services from Possability. This
person will use effective casse management skills to addresses individual needs and enhance
independence, abilities, community participation and/or quality of life.
Working closely with the our Coordination team, this person will ensure client consultation is regular and
relevant, that Support Plans/Personal Plans are current, reviewed and evaluated on a regular basis, and
will be required to investigate, and document client complaints.
This person will act as a role model and mentor to staff by promoting a positive, fun and flexible approach
to client support, ensuring our clients have a strong voice, are given opportunities to make informed
choices, and are consulted in decisions that affect them as individuals and are supported to access
appropriate services.
Operating Context
This position is directly responsible to their immediate manager for all aspects of service delivery and
organisational management for projects and/or programs under their control.

The employee is required to set priorities
and monitor work flow in the areas of responsibility, exercise judgment and initiative where procedures
are not clearly defined, provide expert advice to employees classified at lower levels and/or volunteers,
assist with the preparation of program budgets in liaison with management and to effectively prioritise
the workload and maintain a high quality of service.
Autonomy
The position works under limited direction and has the authority to exercise a degree of autonomy within
the key responsibilities and duties and will be required to exercise initiative and judgment where practices
and direction are not clearly defined. Any issues falling outside these areas are to be referred to the
employee’s manager.
Role Accountabilities

Ensure clients of Possability have a strong voice, are given opportunities to make informed choices,
are consulted in decisions that affect them as individuals and are supported to access appropriate
services;

Undertake
effective
complaint
resolution
processes,
including
the
provision
of
ongoing
communication and feedback to internal stakeholders in regards to current issues and what action
is recommended;

Deliver case management services that encompass an intake process, supported accommodation
placements, new referrals, client need assessments, registering new clients through the NDIS
portal,
matching
services
available
with
client
need
and
the
development
of
case
management/support plans;

Ensure individual case management, support plans and personal plans are current, monitored and
reviewed and meet relevant reporting standards;

Assist the Coordination team and Key Workers with attendance and reporting documentation in
regards to client appointments and case conferences, GAB hearings and other relevant meetings
with external stakeholders and families

Conduct Client Skills Assessments (CSA’s) and provide reports to the Coordination team and
associated health professionals when required

Undertake responsibility for minor to moderately complex projects, including research, planning,
coordination, implementation, delivery and administration

Exercise a high level of interpersonal skills in dealing with internal and external stakeholders

Ensure that the service provided is in keeping with the Disability Service Standards and the policies
and procedures of the Organisation.

Qualifications and
Experience
An appropriate Diploma or Degree qualification, which is recognised by

Previous experience in a relevant industry, service or an equivalent level of
expertise and experience to undertake the range of activities required
Level of Expertise

Current unrestricted Tasmanian driver’s license

Demonstrated ability to build strong and mutually beneficial relationships using
advanced communication and interpersonal skills

Intermediate problem solving, consultation and negotiation skills

Demonstrated ability to work effectively and make informed decisions under
