Please bring this sponsor form to the walk and present it at the reception point. You will be given a numbered card with emergency contact number on. 3. When you have finished walking return to reception point and the sponsor form will be returned certified that walk has been completed. 4. On completion of the walk, sponsorship monies may be collected from the individual sponsors. 5. When all monies are collected, please ensure that the SPONSOR MONEY and THIS FORM are then passed to the Poppy Appeal Organiser of the Branch on whose behalf you have walked. 6.
