SLO Coordinator’s Handbook


Can be the school testing coordinator or another designated person as long as it is NOT a classroom
teacher.
List of 14-15 SLO courses with State Number and Name

Contained in the SLO Coordinator Handbook

The SLO assessments in Performance Matters will be named based on the State Names
SLO Teacher Assurance Form – send all Teacher Assurance forms to Dr. Bourassa by May 30,
2015

Appendix A in the SLO Coordinator Handbook – print one for every teacher administering a SLO
assessment.

All teachers administering a SLO assessment should read and initial each statement, then sign the form
and return to the building SLO Coordinator. These forms will be kept at the school until requested by
the District Coordinator

Have teachers complete this form immediately after administration if issues exist. Send the form to Dr.

Matters

All SLO Assessments are contained within the Performance Matters platform
(exceptions are detailed on the 2014-2015 SLO Administration Guide)


Teachers need to use Test Center to access their post assessments
Online Assessments

Teachers should go into their
CENTER

in Performance Matters
and make the assessments available for
online administration


Students should use the following to log on
to the online assessments:
Username: GTID

Password: Password
Each teacher should monitor students to
make sure they are selecting the correct
test. More than one SLO assessment may be
visible to the students.
Bubble Sheet Assessments

Bubble sheets and test booklets can be
printed from Performance Matters.

If the PDF format is awkward, let Dr.
Bourassa know.

When possible always try to print the preslugged bubble sheets.

Blank bubble sheets are specific to the
assessment and are coded for that
assessment. They can not be used for
different assessments.

SLO Post Assessments will be used for Final Exams.
Scores in Infinite Campus

Codes in IC for missing student scores

-1 = student enrolled after the 65% window cutoff

-2 = student is GAA

-3 = student is Access

-4 = student dropped course before assessment given

-5 = student has pre-measure in another section of a course with the same root course number in school
or district

-6 = student withdrew before test administration window

-7 = hospital/homebound or IEP’d home

-8 = student refuses to attempt assessment

-9 = Other (explanation will be required, please keep a record of the specific explanation available if
requested.)
