Plan For Change
By Group 5
Identified problem: Obesity
Ineffective Health Maintenance
The people of Grand Traverse
County have a lack of familiarity
with neighborhood resources as
evidenced by higher than average
obesity rate, cardiac disease rates,
diabetes mellitus disease, and
malignant neoplasms.
Intervention
Improve access to
health maintenance
activities
 Make sure patients
are aware of offered
activities and
availability

Munson Medical Center
Community agency in charge of project
Rationale: The mission of Munson Healthcare is to
both “provide superior quality care and to promote
community health" (Munson 2008).
Outcome Measures
Some expected outcomes may include :
• Client verbalizes two measures necessary to achieve weight reduction
• Client demonstrates appropriate selection of healthy meals or menu planning
toward the goal of weight reduction
• Client begins an appropriate program of exercise by choosing two activities to
perform at least three times per week for six weeks
• Client is able to make positive dietary changes as evidences by utilization of
available resources
The client will list the six food groups of the food pyramid
and describe the correlation between food and exercise.
Food Pyramid
The client will have fun while learning about nutrition by
participating in learning activities.
Given a food label, the viewer will list the carbohydrate/protein/fat/calories of a
serving size of a specific food.
Nutrition information helpful for
Your own nutritional requirements
Serving sizes
Special diets
low sodium
low fat
low potassium
high fiber
The viewer will describe how portion sizes have changed over the last 20 years
and the increased caloric intake resulting from that change.
How portion size has changed
Recommended Daily Allowances
(RDA’s) for a 2000 calorie diet
Grains – 6 ounces
Fruits - 2 cups
vegetables – 2 ½ cups
Dairy products- 3 cups
Meats/beans – 5 ½ ounces

Nutrition Is Important to Good Health
Morbidity and mortality are linked to a poor diet and a
sedentary lifestyle
Almost 2/3 of Americans are overweight
> 50% do not get enough activity
Diet habits influenced by :
•Family
•Food preparation
•Eating behaviors
Eating Habits
Poor eating habits
develop in childhood
•60% of kids eat more
than RDA of fat
• Less than 20% of kids
eat Recommended Daily
Allowances of fruits and
vegetables
“Healthy People 2010” Goals
•Increase the number of healthy weight
adults (BMI >18.5 < 25)
•Decrease obesity from 23% to 15%
•Increase fruits and vegetables people eat
on a daily basis
•Eliminate differences in a healthy lifestyle
based on socio-economic status
Benefits of a Healthful Eating Plan
Decreased risk of chronic diseases
Type 2 diabetes
Hypertension
Certain cancers
Colon
Breast
Uterine
Kidney
Esophagus
Decreases risk of overweight and obesity
Decreased risk of being micronutrient d
Activity is important to maintain a
healthy weight
Studies show that lack of physical activity during
childhood may lead to health problems in adulthood like
obesity, heart disease, and Diabetes type 2.
Maintaining weight
- Your weight will stay the
same when the calories
you eat and drink equal the
calories you burn.
Losing Weight
- You will lose weight
when the calories you eat
and drink are less than
the calories you burn
Gaining Weight
- You will gain
weight when the
calories you eat
and drink are
greater than the
calories you burn.
Health Risks
Risk of obesity for:
Caucasian adults – 25%
Blacks and Latinos 30-50%
Native Americans – 60%
Health Promotion
Focus on education because morbidity and mortality
are directly related to lifestyle and eating habits
Importance of Fruits and Vegetables
Diets high in fruits and
vegetables significantly
reduce the risks of:
•Stroke
•Heart disease
•Several types of cancer
Abstract
Obesity is a health problem that the United States has struggled with for
decades, and there are many health problems associated with obesity. The
obesity epidemic is decreasing quality of life for millions and driving up the cost
of healthcare .
Obesity is a modifiable risk factor, and when people adopt healthy lifestyles
and lose weight, they can reverse many detrimental health problems.
By improving access to health maintenance activities and providing nutrition
counseling on portion sizes and food label reading, clients will have improved
knowledge to increase healthy behaviors and promote weight loss with an
eventual decrease in risk factors related to obesity
