Before mailing your application, please verify that you have completed, signed, and included
all of the items on the following checklist. To be considered a complete application, all items
must be received no later than May 12th, 2014. Be sure to make copies of all submitted

REFUNDS: Full refunds will be given only under the following circumstances:
(1) A request is received in writing for a refund NO LATER THAN MAY 12th, AND
(2) The participant’s space is filled by another individual, paid in full.
We apologize for any inconvenience but no refunds will be issued after May 12, 2014.

GENERAL INFORMATION
Logistics
Handlers should expect to drive to/from the training locations and will need to provide their
own means of transportation. Handlers are responsible for their own food, water, gear, and
other necessities.
Emergency Information
A medical-veterinary safety document will be provided to every participant on the first day of
the seminar. There are both human emergency centers and animal emergency facilities in
the Amherst area as well as a paramedic-level ambulance service on the campus.
Events and Other Information
This is a certification workshop and organized training seminar. This does not preclude
handlers from training or self-organizing, however training may not occur at the facilities
where testing elements are being held. Handlers are required to comply with all rules, laws,
regulations, or other constraints during the testing and will be responsible for bringing and
utilizing all required gear.
Maps will be provided and handlers are responsible for ensuring they are at the test locations
at the required times. The schedule for testing elements will be determined by the evaluators
and organizers. Handlers are responsible for all fees for failed elements as outlined in the
NAPWDA By-Laws and Certification document. Handlers are also responsible for providing
all prerequisite documentation to the evaluators. Do not submit prerequisite documentation
with your application.
Cost
The cost to participate is $280 per team; a lunch meal will be provided by MAPWDA each
day of the week. The registration payment will cover the NAPWDA trainers’ travel and per
diem fees. All other meals, gear, fuel, lodging, and supplementary expenses are the
responsibility of the handler. Handlers with multiple dogs wishing to certify should contact a
seminar coordinator. A completed application will be required for each dog and handler.
Lodging
The Hampshire Hospitality Group has two facilities in the greater Amherst area available to
seminar attendees. Both will allow patrol dogs inside the rooms during your stay. Please be
sure to secure your room reservation quickly as lodging options become more limited
as college graduation ceremonies near. Rates, rules, and info may be found on the
attached lodging sheet.
Seminar Check-in and Orientation
If attending this year’s seminar, please plan to arrive at the UMass Amherst Police
Department on Monday, May 19th at 0800 hours. The GPS address is 585 East Pleasant
Street, Amherst, MA 01003. Ample parking is available in the police parking lot and along
the roadway across from the Amherst Fire Department. If you need detailed directions,
please contact Officer Varosky at the number listed below.

Lunch Meals for Seminar Participants
Again this year, seminar participants may opt for an on-campus meal program through the
University of Massachusetts Dining Services. The location is Franklin Dining Hall and
provides a variety of hot meals, grab-n-go foods, and specialty items. Vegan, vegetarian,
and other dietary needs will be addressed by the staff if necessary.
Included with your seminar registration payment is one (1) lunch per day at Franklin Dining
Hall. The lunch coupons will only be accepted at this dining hall and are not valid at offcampus eateries. Details and directions will be provided to attendees during Monday
morning orientation.
Breakfast and dinner meals may also be obtained through Franklin Dining Hall. Please note
that the purchase of these meals will be the responsibility of the seminar attendee. Sample

Jack Schonely Lecture
Additionally, we welcome Jack Schonely (L.A.P.D.) who will present a one-day seminar on
“Suspect Tactics and Perimeter Containment”. Jack has participated in over 2,000
perimeter containments during his career, and he has seen many successes and failures. He
has shared his experience with thousands of law enforcement officers across the United
States and Canada instructing on this topic. He is the author of Apprehending Fleeing
Suspects; Suspect Tactics and Perimeter Containment© and has been published in
numerous law enforcement publications.
Jack Schonely will instruct on Friday, May 23, 2014 at the UMass campus. The curriculum is
open to all law enforcement officers to include K9, SWAT, field supervisors, air crew, watch
commanders, and first line patrol responders. For those officers wishing to only attend Mr.
Schonely’s seminar, please see the MAPWDA website for application forms.
The tuition for Mr. Schonely’s seminar is included in the K9 Summer Seminar fee of $280.

Release / Indemnification and Hold Harmless Agreement

lands of the University of Massachusetts at Amherst (hereinafter referred to as “UMASS”) exposes me
to the risk of personal injury, death, or property damage. I hereby acknowledge that I am voluntarily
participating in the MAPWDA K9 Patrol / Narcotics Certification and Workshop (“Activity”) and
expressly agree to assume any such risk.
I understand that I am not required to participate in this Activity, but I want to do so, despite the
possible dangers and risks and despite this Release.
In consideration for being permitted to participate in this training, I hereby release and forever
discharge UMASS, the University of Massachusetts Police Department, the Commonwealth of
Massachusetts, the town of Amherst, the town of Hadley, the Massachusetts Police Work Dog
Association, and all their respective officers, employees, agents, Board of Trustees, and volunteers
from any and all liability, claims, demands, damages, actions or causes of action arising from or by
reason of any injury to or death to me or any person, or any damage to or loss of personal property
resulting from or arising out of any accident or occurrence in connection with my participation in the
