Please read the following consent and liability waiver. All players (full-time or replacements) willing to participate in the Bytown Touch Football League must
read this form before having the right to play in any team. All players (full-time or replacements) must fill out the chart including their full name, postal code,
date of birth, telephone and email address. All players (full-time or replacements) must sign this form. In doing so, players (full-time or replacements) fully
agree to all terms and conditions of the following statements:
1. ASSUMPTION OF RISK: I understand that participation in or attendance at any Bytown Touch Football league game involves inherent risks and dangers of accidents,
property loss or damage, serious personal and bodily injury, death, and severe social and economic losses. These may result not only from a participant’s or another’s
own actions, inactions, or negligence, but the rules of play, or the condition of the fields or equipment. Furthermore, there may be other risks not known to me or
foreseeable at this time. I understand and I have considered and evaluated the nature, scope, and extent of the risks involved to all participants, and I voluntarily and
freely choose to assume all of these risks. Finally, I understand and warrant that I will not participate in any Bytown Touch Football game or event unless I am and
continue to be in good health and have no physical condition that would prevent me from participating in these games or events. I take full responsibility and release the
Bytown Touch Football League of any risk in participating in any Bytown Touch Football League game or event if I suspect being ill or injured whether it be related
directly or indirectly to any Bytown Touch Football League game or event.
2. RELEASE FROM LIABILITY: I understand that participants fully and forever release and discharge the Bytown Touch Football League, its subsidiaries, president,
vice-president, officers, employees, volunteers, sponsors, advertisers or operators of the fields and equipment and all others involved in any Bytown Touch Football
League event (the “Released Parties”), from any and all injuries (including death), losses, damages, claims (including negligence claims), lawsuits, and any other liability
of any kind, of or to a participant, his/her property, or any other person, directly or indirectly arising out of or in connection with a participant’s attendance at a Bytown
Touch Football League game or event even if it is due to the negligence or other fault of the Released Parties.
3. COVENANT NOT TO SUE: I understand that participants will not initiate any lawsuit, court action or other legal proceeding against the Released Parties, nor join or
assist in any prosecution of any claim for money damages which anyone may have, on account of injuries (including death), losses, or damages sustained by a participant
or others, in connection with a participant’s attendance at a Bytown Touch Football game or event.
4. INDEMNITY: I understand that participants will hold harmless, indemnify, and reimburse the Released Parties from and for any sums, costs or expenses (including
attorney fees) incurred by any of he Released Parties or paid by them to any person (including a participant or his/her insurers) in connection with any accident, injury
(including death), loss, or damage sustained by a participant or others in connection with a participant’s attendance or involvement in any Bytown Touch Football League
game or event.
5. NO INSURANCE; MEDICAL EXPENSES; MEDICAL CARE: I understand that the Bytown Touch Football League and its employees do not provide a participant
with any insurance, either life, medical or liability, for any illness, accident, injury, loss, or damage that may arise in connection with a participant’s attendance at a
Bytown Touch Football game or event. I authorize and permit the Bytown Touch Football League, its agents and employees to administer first aid to the a participant,
emergency transportation, and any other medical treatment performed by physicians, paramedics, and other medical personnel, in the event of any illness, accident or
injury to a participant.
6. RULES, REGULATIONS AND RESPECT: I agree to abide and respect the rules and regulations of the Bytown Touch Football League in accordance with its rules,
regulations and by-laws. I agree to respect the decisions and penalties given by referees and employees of the Bytown Touch Football League as well as understand that
my right to play in the Bytown Touch Football League is dependant on a participant’s and his/her team’s respectful play and conduct. I understand that The Bytown
Touch Football League, its president, vice-president, officers and any employee can revoke a participant’s right to participate or play in any or all games or events if a
participant does not respect League rules and regulations, referee and employee decisions and play the game in a respectful manner,
7. PHOTOGRAPHY AND MEDIA: I hereby grant to the Bytown Touch Football League, the exclusive right to permit or authorize any person, firm or corporation to
capture and make use of any photographs, videos or television broadcasts, as well as the reproduction of my name in connection with my participation in the Bytown
Touch Football League and these may be used, reproduced, distributed and otherwise disseminated by the Bytown Touch Football League directly or indirectly in any
manner they desire.
I HAVE CAREFULLY READ THIS AGREEMENT AND FULLY UNDERSTAND ITS CONTENTS. I AM AWARE THAT THIS IS A RELEASE OF
LIABILITY AND A CONTRACT BETWEEN MYSELF AND THE BYTOWN TOUCH FOOTBALL LEAGUE, ITS AGENTS, SPONSORS AND
EMPLOYEES, AND I HAVE SIGNED IT OF MY OWN FREE WILL.
