can be used by the MATES National Honor Society Faculty Council to assist with the
selection process. Completion of this form DOES NOT guarantee selection into the
MATES National Honor Society. You may transfer this electronically to fit the
information into the form.

II. Co-curricular school activities – List all activities (also major accomplishments in

III. Leadership Positions – List all elected or appointed leadership positions held at
MATES, community, sports or work activities. Only those positions in which you
were directly responsible for directing or motivating others should be included.
For example: elected student body, class or club officer; committee chairperson;
team captain; newspaper editor; work area manager; or community leader. List the
year only that you served a leadership position. Independent research presented
outside MATES should be included in this section

IV. Outside Service/Community Activities – List service activities in which you have
participated. These can be service projects done with a group either inside or outside
of school, or done as individual projects (performing a type of service). Generally,
service activities are those which are done for, or, on behalf of others (not including
immediate family members) for which no compensation (monetary or other) was
given. For example: religious groups, clubs sponsored outside of school, Boy
or Girl Scouts of America, or community art endeavors. Do not repeat
participation already listed above in section I. You can include information in
section II, only if it is outside or MATES and note described in section I. Please
list the name of an adult supervisor who can verify your participation in each

VI. Essays. Please provide responses to the following two questions below using a
separate sheet of paper. Please be sure that you use a font no less than 10 pt and no
greater than 12 pt (Times New Roman or Arial font types are acceptable). Be sure
that your essays are double spaced with you name on the top of each page. Essays
are to be no more than 300 words (typed) each.
1. If you were selected to the MATES NHS Chapter, how would you contribute
to the continued growth and success of our Chapter?
2. The National Honor Society encompasses four aspects of a person including
character, scholarship, leadership and service. In terms of character: “A
candidate will exemplify the guidelines and policies of MATES with
outstanding conduct and behavior. Some of the qualities associated with
character include: trustworthiness, fairness, responsibility, self-discipline,
citizenship, and concern for others.” Please write a brief essay on the ways
that you display positive character in both school and within your community.
