Application Directions and Other Pertinent Information
1. Do not put your name on the application. Use only your school identification
number. Your signature on the essay form is the only place your name should
appear.
2. Print neatly or type the student activity information form.
3. Your essay must be typed.
4. Make sure you ask 3 teachers for recommendations. Give each teacher enough time
to fill out the forms, at least a week. Place your ID number on the forms before you
give them to the teachers.
5. Enclose a copy of your school schedule with the completed application.
6. Do not staple any of the papers together. You may use a paper clip.
7. Please place your application and essay in a pocket folder with your ID number on it.
8. Please be advised, if you are selected as a member of the National Honor Society, a
one time due of $20 will be requested.
9. If selected as a member of the National Honor Society, you are required to complete
10 hours of volunteer service per marking period.
10. If selected as a member of the National Honor Society, it is an important service
obligation to be part of the meeting agenda and participate in service projects
throughout the year.
