Agency: Services Provided
The following report is used to report on all services provided by an agency to clients with records in the
Client Registry.
This report includes demographic information which may be useful for demonstrating how services were
distributed to both majority and under-represented populations.
Criteria Option 1: Agency Name
Use the Agency name as listed in the Resource Database. To find the exact title of your agency listing, go
to the Resource Database search function at:
Agency Tools > Agency, Program & Service Resources > Advance Search
Criteria Option 2: Service Profiles ID
Use the Service ID number(s) (without the “S”) To find the service ID numbers for your agency, go to the
Resource Database search function:
Agency Tools > Agency, Program & Service Resources > Advance Search
To run reports on multiple services provided by your agency use an “OR” connector between each logic
statement.
