AUTHORISATION FOR RELEASING A CONSULTANT-LEVEL POST Guidance Notes FILLING OUT THE FORM The Originating Manager information and Sections A, B C and E must be completed for ALL consultant-level posts (NHS and QMUL employed). Section D is required only for QMUL employed posts. ORIGINATING MANAGER INFORMATION – To be completed for all posts The originating manager will have oversight responsibility for ensuring accurate completion of the form, attachment of all relevant documents and acquiring signatures. For NHS posts: The originating manager will usually be the relevant CAU General Manager. Contact BLT Medical Recruitment to assign the BLT post ref number. For QMUL posts: The originating manager will usually be the relevant Institute Manager, who will assign an SMD Institute ref number. SECTION A – To be completed for all posts 1.1 Indicate 100% funding source, or show % split if from more than one source. If Research Grant funded, provide details of grant awarding body/ies. Gross salary includes basic salary, additional allowances and all on-costs (pension, NI, etc). 2.3 Tick one box only. 2.4 Fixed term contracts require by law an ‘objective justification’. QMUL posts only: indicate type of academic post (TS, PI, TR). 2.5 If post is part time, indicate the % of full time, FTE, hours per week, weeks per year, etc. 2.6 All Consultant level posts require a CRB check. The duties of the post will dictate whether an Enhanced or Standard disclosure is required. For advice and guidance contact QMUL HR Dept or BLT Medical recruitment. 4.1 All posts should be advertised internally in both organisations. All Consultant level posts must be advertised via at least two different media, one of which must be a printed journal. Only NHS-employed posts can be advertised via the jobs.nhs.uk online application system. 4.2 Indicate as many details of panel members as possible. This may help to speed up interview arrangements. In particular, please ensure that all proposed panel members have received up-todate equality and diversity training in relation to recruitment and selection. SECTION B – To be completed for all posts 5 No post may be advertised with more than ten PAs in the standard weekly job plan. 6 The nature of any additional PAs above 10 should be indicated clearly as either Academic or Clinical and detailed justification for payment must be given. Full details of all duties will be included in the job description, a copy of which must accompany this form (see 18.1). For QMUL employed academic posts, further detailed information regarding education and research is required in Section D. 14 Please provide as full and accurate figures as possible in each relevant category, in particular differentiating between existing and new funding sources. SECTION D – To be completed for QMUL posts only 15 Budget codes for salary costs, recruitment costs, and work permit applications must be provided and authorised by Management Accounts and/or Joint Research Office. 17 Please discuss preparation of the investment appraisal spreadsheet with your Institute Manager. SECTION E – To be completed for all posts 18 Ensure all relevant documentation has been attached (hard copy and electronic). Indicate relevant Royal College from which approval is required. This will usually be sought by QMUL HR Dept (for QMUL posts) or BLT Medical Recruitment (for NHS posts) but please attach copy of approval if already obtained. 19.1 All signatures should be obtained from all relevant persons before the form and attachments are forward on in hard copy and via email.
