The Water Services Trust Fund (WSTF) has come to the conclusion that your performance as the
(Engineering/Social/Financial) Field Monitor for (Project Names & Project Types) projects has
not been satisfactory. The WSTF has compiled an evaluation report regarding your
performance, specifying the issues observed. It is on the basis of this evaluation that the
Company, in line with Clause 14A(d) of the Field Monitoring Contract, has decided to relieve
you from all your duties with effect after 60 days from now.
You are therefore instructed to immediately surrender all the Company property in your
possession to the undersigned.
