We accomplish this in four ways: Patient Care Blythedale is a national leader in the diagnosis and treatment of children with disabling and complex medical conditions. Research Blythedale is a research center dedicated to discovering ways of improving the care of children with special needs. Teaching Blythedale is training the next generation of healthcare professionals in the diagnosis and treatment of children with complex medical needs. Advocacy Blythedale advocates tirelessly at the federal and state levels to ensure that the highly specialized needs of all children are met. We serve children without regard to religion, race or ability to pay. Although we treat children from any locality, we consider the greater New York metropolitan area, Hudson Valley and Connecticut to be our primary communities. d Dear Friends, We are very proud to share the 2013 Annual Report of Blythedale Children’s Hospital. Over the last decade we brought our dreams to fruition with the opening of our new inpatient hospital, The Stavros Niarchos Foundation Center for Speech & Audiology, and most recently with the opening of our new Day Hospital and Early Childhood Center. Yet we look at our decade-long building program not as an end, but rather as the beginning of a new journey that will enable us to serve more patients, develop new programs, and create new services to meet the critical needs of infants, children, and adolescents who desperately need Blythedale. In order to continue to bring our broad vision to reality, we need your support. In our current healthcare economic climate, the reality is that reimbursements alone cannot cover the full cost of care for a child with extensive and complex medical needs. Blythedale treats patients regardless of financial ability and a staggering 70 percent of our patients are Medicaid-dependent. The generosity of our friends and community fills in the critical gaps and allows Blythedale to grow and develop an ever-broadening scope of services. With more than 120 years of experience, Blythedale is a nationally recognized leader in developing innovative, multidisciplinary inpatient programs. As one of only 19 specialty children’s hospitals in the United States, Blythedale has the combined staff, expertise and resources – including the largest pediatric therapy department in New York State – to provide exceptional comprehensive care for medically complex children and support for their families. Blythedale cares for children with a wide range of complex and often catastrophic medical and rehabilitative conditions, including complex respiratory disease, traumatic brain injury, heart disease, cancer, neuromuscular disorders, and other serious genetic conditions. Through its inpatient and day hospital programs, Blythedale treats more than 300 children daily, making it one of the largest providers of children’s health services in the State. Ensuring care of the whole child, Blythedale is the only hospital in New York State with its own public school on-site serving its patients. 2013 Highlights 19% Diagnostic Categories 31% 19% 31% 21% 21% 29% 31% General Rehabilitation 29% Ventilator Weaning & Respiratory 21% Neuro TBI/ABI 30% 19% Medically Complex 2 • In December, Blythedale celebrated the opening of its new $15 million Day Hospital and Early Childhood Center. The 8,000 square-foot Day Hospital features new exam rooms, treatment areas, feeding therapy rooms, and a spacious audiology booth. The newly renovated Early Childhood Center includes four refurbished classrooms to facilitate the program’s growth. This project was supported by a generous $250,000 grant from The Thomas and Agnes Carvel Foundation. • In May of last year, Blythedale welcomed Dr. Scott M. Klein, M.D., M.H.S.A. as the Hospital’s new Chief Medical Officer and Chief of Pediatrics, and Jill M. Wegener, M.S.N., R.N., C.C.R.N. as its new Chief Nursing Officer. • Blythedale recorded 594 unique patient admissions in 2013, totaling 24,856 inpatient days. Approximately half were admitted for comprehensive acute inpatient rehabilitation (including recovery from complicated surgery, stroke, multiple trauma, and rheumatologic and neurologic crisis), while nearly a third were admitted for respiratory care or ventilator weaning. The average length of stay was 50 days. The Hospital received admissions from 60 hospitals and leading academic centers in 2013, including New York Presbyterian (Columbia and Cornell), Westchester Medical Center, Montefiore Medical Center, Steven and Alexandra Cohen Children’s Medical Center of New York/North Shore LIJ Health System, Mount Sinai, Jacobi Medical Center, Hospital for Special Surgery and NYU/Bellevue. • In 2013, Blythedale expanded its Pulmonary Hypertension program, treating 27 medically fragile infants with a complicated treatment protocol including oral and inhaled medications, and intravenous or subcutaneous treatments. • Blythedale continued contracting with school districts in Westchester and Rockland counties to provide on-site Physical Therapy (21 schools in 8 districts), Occupational Therapy (25 schools in 6 districts) and Speech Therapy (20 sites in 9 districts) for students. In addition, the Hospital provided Audiology services in three districts. • Last November, Blythedale implemented a new medication management safety initiative. Bedside Medication Verification (BMV) enables staff to scan patient and medication data into our electronic medical record to ensure that the right patient receives the right medication. Since implementation, the Hospital has consistently achieved a 95% scanning compliance rate. Research The vision of the Blythedale Research Institute is to be a center of excellence for “bench to bedside” research that will shorten the interval from discovery to clinical implementation of new and improved treatments in the care of children with complex, disabling illnesses. The Research Institute will serve as a model of new clinical treatments and their outcomes for children with complex illnesses through meaningful collaborations within the Blythedale Children’s Hospital community, as well as with similar institutions across the country and around the world. Blythedale conducts select studies that promise to have a significant impact on treatment modalities for children with complex medical and physical challenges. Our robust Research program is a natural outgrowth of our commitment to children and our culture of innovative clinical and multi-disciplinary care. Blythedale’s Scientific Advisory Board, comprised of prominent scientists and pediatric faculty, provides feedback on our research studies. 2013 Highlights • Robotic therapy studies continued as a main research area in 2013. Blythedale completed two collaborative studies with MIT, Indiana University, Rancho Los Amigos and New York Medical College that investigated how important variables in motor learning affect the improvement seen in motor function following robotic therapy. Last year, researchers began studying robotic therapy using a new robot designed at MIT—the Anklebot. This robot is our first to study robotic therapy in the lower extremity. In addition, we purchased a robot for hand therapy that can be programmed to concentrate on function of specific fingers. • In preparation for a major study planned with Cornell, Columbia, Burke and Blythedale, Blythedale’s research team trained and was certified in transcranial direct current stimulation (tDCS). Through the Brain Injury and Repair Research Study, we are piloting a model of collaboration with Burke Medical Research Institute that will help speed the process of translating research into patient care. In addition, we have developed a database of studies in the New York area that may be useful to different patient populations that we see. • Blythedale’s physical therapists completed a study of safe walking speed using the Balance Master. A paper describing their results is in preparation. Physical therapists are planning a study using the Walk Aide, which uses functional electrical stimulation to treat footdrop, to help patients climb and descend stairs. • Since the formal research program began in 2006, Blythedale researchers have published 37 abstracts and papers. • As part of a generous grant, The Stavros Niarchos Foundation supported clinical research at the Hospital in 2013, including studies on the efficacy of robot-aided therapy in children with brain injuries as it relates to the recovery of mobility in the arm and wrist. 3 2013 ANNUAL REPORT Teaching & Training Education is an important component of the mission of Blythedale Children’s Hospital. Serving as a training ground for the next generation of pediatric specialists, more than 100 colleges and universities throughout the region rely on our Hospital to provide clinical training to scores of professional students in a wide range of fields. Our staff serves as faculty at top colleges and universities, sharing their expertise in areas including medicine, nursing, respiratory therapy, physical therapy, speech therapy, occupational therapy, child life, social work and hospital administration. Blythedale is also committed to educating parents to be competent caregivers, helping them every step of the way as they grow into their new role as caregivers of a child with complex medical needs. In addition, we are proud to serve as a vital resource to our community, offering programs tailored to every aspect of child health. 2013 Highlights • Thirty-seven medical residents and fellows spent at least a portion of their residencies at Blythedale in 2013. These physicians were graduates of the leading medical schools in the U.S. and abroad, and completed rotations in Rehabilitation Medicine, Orthopedics, Pulmonology and Psychiatry. Blythedale also hosted 25 medical students and observers from several major teaching hospitals, including New York Medical College, New York Presbyterian - Columbia University Medical Center, and NYU Langone Medical Center. These students/observers were assigned to physicians and therapists at Blythedale who complemented the student’s interests. • In addition to providing clinical training experiences for 225 nurses from eight schools in 2013, the Hospital provided clinical internship opportunities to students in a wide range of departments. • Formal teaching, in the form of Grand Rounds, was presented by expert physicians from our major referring institutions, including New York Presbyterian Hospital, Maria Fareri Children’s Hospital, Montefiore Medical Center, BronxLebanon Hospital Center, NYU School of Medicine, Columbia University College of Physicians and Surgeons, and Morgan Stanley Children’s Hospital on topics including Management of Neurogenic Bladder, Early Onset Scoliosis, Complex Regional Pain Syndrome and Children’s Environmental 4 Health Challenges. In addition, the Hospital held a 10-part lecture series on Palliative Care for its staff. Topics included pain and symptom management, ethical and legal issues, end of life care for families and caregivers, comprehensive therapies, communication and more. • Last year, Blythedale in coordination with the Westchester Regional EMS Office, Westchester County DES, and NYS EMS for Children, developed a special pediatric continuing medical education (CME) series for first responders of the Westchester EMS region. The series covered subjects vital to first responders, including traumatic brain injury, autism, and technology and care of the infant or child. These courses were presented by clinical experts from Blythedale, including physicians, nurses and therapists. • In 2013, Blythedale continued its vital community service as a certified Training Center for the American Heart Association (AHA) in both Pediatric Advanced Life Support (PALS) and Basic Life Support (BLS). In addition to bringing these vital program out into the community, the Hospital offered a variety of courses on-site. In 2013, 4,528 people were trained in CPR, PALS and first aid through Blythedale’s Training Center. In addition, 110 parents and caregivers were trained last year. We have 120 instructors registered with our Training Center. Advocacy Advocating on a federal, state and local level for programs that benefit children with special healthcare needs is paramount to our mission. Blythedale is also committed to providing health-promotion services to minimize the incidence of disease among all children. We continue to demonstrate our commitment to reducing the number of preventable injuries and deaths among children through our participation in numerous injury-prevention-related activities. We provide CPR training and car seat checks, and make presentations on bicycle and water safety at local health fairs. Blythedale’s medical and clinical professionals serve as an important resource, speaking on a wide range of children’s health topics to corporate groups, community organizations, and school and parent groups. 2013 Highlights • In 2013, Blythedale worked actively with the nationally based Children’s Hospital Association to advocate for the implementation of federal healthcare reform so that it adequately addresses the special health needs of children, particularly medically fragile children. • Blythedale also participated in the New York State Workgroup on Medically Fragile Children, advocating for policies that would protect medically fragile children in the State’s transition to Medicaid managed care. • Hospital leadership remains in frequent contact with United States Senators Charles Schumer and Kirsten Gillibrand, and Congresswoman Nita Lowey, Congressman Eliot Engel and Congressman Sean Patrick Maloney. Last year, the Hospital hosted Congressman Maloney for a tour, and was also visited by NYS Senator Andrea Stewart Cousins, NYS Assemblyman David Buchwald, NYS Assemblywoman Shelley Mayer, NYS Assemblyman Steven Otis, and Deputy Westchester County Executive Kevin Plunkett. • Last year, Blythedale participated in a workgroup of local hospitals convened by the Westchester County Department of Health. Promotion of healthy women, infants and children by increasing breastfeeding was identified as a community health care priority by the group. Blythedale embarked on a plan to support this initiative by promoting awareness of the benefits of breastfeeding with staff and patient families, enhancing staff knowledge through annual in-service programs, and supporting lactating mothers during their child’s hospitalization at Blythedale. • Funded by a generous grant from Kohl’s Department Stores, Blythedale’s “Eat Well, Be Well” program teaches healthy eating habits to children in kindergarten through eighth grade by providing general nutrition guidelines to students, parents and school faculty. In 2013, the program reached more than 16,000 students in 23 school districts, bilingual materials were developed, and staff was increased to meet the growing demand for programming. • Last year, Blythedale served as the local coalition leader for the National Safe Kids campaign.
