FUROSEMIDE CLASSIFICATION Loop diuretic TRADE NAME(S) Lasix DESIRED EFFECTS Potent and rapid diuretic; increased water excretion and venous dilation; reduces ventral venous pressure MECHANISM OF ACTION Inhibits sodium and chloride reabsorption in the kidney INDICATIONS • • Pulmonary edema Congestive heart failure CONTRAINDICATIONS • • Hypotension (SBP<100 mmHg) Hypersensitivity ADVERSE REACTIONS • • • • • • • • • Volume depletion (dehydration) Hypotension Potassium depletion Rash Headache Deafness Cardiac arrhythmias Chest pain Allergic reactions DRUG INTERACTIONS • Incompatible with diazepam, diphenhydramine, and thiamine PRECAUTIONS • • • Pregnancy Protect from light Give slowly to prevent transient deafness SPECIAL CONSIDERATIONS Patients taking furosemide may require a larger dose to achieve desired effects in the acute setting. However, IV doses of furosemide are not subject to first pass metabolism making its effects greater. Cardiac monitoring should be performed. The onset of venous dilation is approximately 5 minutes and the onset of urine production is approximately 10 minutes.
