The RBAC 4-A reserves the right to accept or reject any bid, to annul the bidding process, to reject all bids at any time prior to contract award without thereby incurring any liability to the affected bidder/s and to waive any required formalities therein and to award the contract to the bidder whose proposal as evaluated is most advantageous to the Bureau.
The RBAC 4-A assumes no obligation whatsoever to compensate or indemnify the bidder for any expenses or loss that may be incurred in the preparation of the bids, nor does it guarantee that any award will be made.

The Eligibility Envelope shall contain the following:
Class “A” Documents
LEGAL DOCUMENTS of Trade and Industry (DTI) for sole proprietorship, or Cooperative Development
Authority (CDA) for cooperatives or any proof of such registration as stated in the
Bidding Documents;
Valid and current Mayor’s permit issued by the city or municipality where the principal place of business of the prospective bidder is located;
TECHNICAL DOCUMENTS
FINANCIAL DOCUMENTS authorized institutions, for the preceding calendar year which should not be earlier than two(2) years from the date of bid submission, showing among others the total and current assets and liabilities.
Class “ B” Documents the absence of a JVA, duly notarized statements from all the potential joint venture partners stating that they will enter into and abide by the provisions of the JVA in the instance that the bid is successful shall be included in the bid. Failure to enter into a joint venture in the event of a contract award shall be ground for the forfeiture of the bid security. Each partner of the joint venture shall submit the legal eligibility documents.
The submission of technical and financial eligibility documents by any of the joint
venture partners constitutes compliance.(a)

Latest Income Tax Return and Business Tax Returns for CY 2011 (electronically filed

BIR Registration Certification, which contains the Taxpayer’s Identification Number
Articles of Incorporation, Partnership or Cooperation, whichever is applicable, including amendments thereto, if any; supply the goods/equipment and Letter of Authority to participate in the bidding by the manufacturer, bidder, who is under an international exclusive distributorship from his respective international exclusive distributor and a notarized copy of exclusive distributor from the manufacturer; and, the most recent or current model; have no defect arising from design, materials, and workmanship. A warranty is required from the supplier, for a minimum period of one (1) year.

1. Price quotation should be inclusive of 5% VAT and expanded withholding tax (EWT) of

Evaluation of the quoted price will be determined on the basis of the total price.
This request for quotation must be sealed and submitted or ready for pick-up on or before the mentioned date in the ITB
Refusal to accept the Notice of Award maybe ground for the imposition of Administrative
Sanction under Rule XXIII of the Revised IRR of RA 9184.
All waste materials is required to be turned over by the contractor to BFP R4A RHQS properly receipted by the end user and duly acknowledged by the OPFM concerned.
Place of delivery: of repaired FT/MV is at BFP RHQS4A Compound, Camp Vicente Lim,
Calamba City, Laguna for initial inspection prior to delivery to recipient end-user.
Seven (7) days observation of repaired FT/MV delivered at recipient end-user is required prior to final acceptance and payment.
