WAIVER OF LIABILITY and ASSUMPTION OF RISK
In consideration for permission to participate in the recreation & fitness activities and facilities offered by Florida International University (FIU) Wellness & Recreation Center - MMC
(Department), on behalf of myself, my spouse, family, heirs, and assigns (collectively Me), I release and hold harmless FIU, the Florida Board of Governors, the FIU Board of Trustees,
the State of Florida, its employees & agents (collectively the University) from liability arising from injury, death, or property loss suffered by Me which may result from the negligence of
the University while using recreational facilities, equipment, or participating in Department-sponsored activities.
I acknowledge that I know, understand, and appreciate the inherent risk of using recreation & fitness equipment and facilities, and participating in the recreation & fitness activities
that I choose. I know that these risks range from minor scrapes, strains, and bruises to significant injuries, such as broken bones, eye injury or loss, concussions, paralysis, and even
death, and may result from my action, others’ actions, or both. By signing this Agreement, I fully assume the inherent risks associated with using the facilities, equipment and
participating in the activities provided the Department, and assert that my use of or participation in them is voluntary. I further agree to abide by the terms and conditions contained
on the reverse page.

IT IS MY INTENTION TO EXEMPT AND RELIEVE THE STATE OF FLORIDA, FLORIDA BOARD OF GOVERNORS, THE FLORIDA INTERNATIONAL UNIVERSITY BOARD OF TRUSTEES,

WRC Membership Information
The FIU Wellness & Recreation Center _ MMC (WRC) provides recreation and fitness opportunities to the FIU community. WRC Membership allows FIU community members the
privilege of using recreation facilities and programs including the WRC, the Tennis Center, Panther Hall & Tamiami Park Pools, the Recreation Field, and to participate in Intramural
Sports, PantherFIT, Adventure Recreation, and other programs.
MEMBERSHIP CATEGORIES
A. FIU STUDENT
• CURRENTLY-ENROLLED FIU STUDENTS with up-to-date payment of the Activity and Services (A&S) Fee are automatically WRC members, pay no additional membership fee, and are permitted to use and participate in WRC activities. Additional fees may be applicable for some programs/services.
B. UNIVERSITY MEMBER
• EMPLOYEES of the University, including full-time Faculty (not adjunct), Administrative, and Staff Employees, are eligible to purchase a WRC membership. FIU employees eligible for payroll deduction may purchase a WRC membership on an annual or semester basis, or through payroll deduction. Payroll deduction membership fees are deducted post-tax and are ongoing. To discontinue the deduction, written notice must be received by the Director of the WRC. Please allow at least 1 payroll cycle for processing start-up/cancellation through the Payroll Office. Employees using the FIU Tuition Waiver should inquire at the front desk about appropriate membership.
• RETIREE/EMERITUS FACULTY are those individuals who, having been employed by the University for a length of time, have officially retired from service and are recognized as such by the Office of Human Resources. Retirees/Emeritus Faculty may purchase a membership on either an annual or semester basis.
• STUDENTS NOT CURRENTLY ENROLLED (SNCE) may purchase a WRC membership for no more than one semester after their most recent semester of enrollment.
• DUAL ENROLLMENT students may purchase a WRC membership for the current semester of enrollment.
C. AFFILIATE MEMBER
• ALUMNI may purchase a WRC membership on either an annual or semester basis. Alumni are defined as graduates of FIU, or individuals who have accumulated 30 or more credit hours from FIU. Alumni must be able to present proof of status (diploma or transcript) with the completed WRC Membership Application Form.
• FIU ALUMNI ASSOCIATION members will receive a discount on the regular Affiliate rate. To qualify, a current-year FIU Alumni Association Membership Card must be presented at time of purchase. General Public individuals who are FIU Alumni Association members also fall into this category.
• The SPOUSE of any of the above categories is eligible to purchase a WRC Membership. The qualifying Spouse member must present proof of marriage and his/her Panther
ID along with the spouse’s completed application form at the time of purchase. The qualifying Employee member may add their spouse to the payroll deduction option.
There are no provisions for dependent (family) memberships.
• PARENTS OF FIU STUDENTS may purchase a WRC membership on either an annual or semester basis. The student must be enrolled for the duration of the membership.
POLICIES
• All current students and WRC members are required to present an activated FIU OneCard photo ID card at all times to enter recreation facilities or participate in programs.
Affiliate members who do not already have an FIU OneCard photo ID may purchase a special Affiliate FIU OneCard ID at the FIU OneCard ID Office (Gold Garage) prior to
use. Call (305) FIU-CARD for hours and current price.
• Membership applications and appropriate payment/payroll deduction authorization must be brought to the WRC Membership Desk. A photo ID must be presented at the time of purchase to validate the transaction. Payment may be made via cash, credit card, FIU OneCard, money order or cashier check – no personal checks.
• Annual memberships are refundable up to two (2) weeks from purchase date (less a $5 service charge). Semester memberships are not refundable.
• Please be prepared to display your WRC membership receipt along with your FIU OneCard ID for up to 5 business days after initial purchase to allow activation of your card. An activated FIU OneCard ID is required after this 5 business day period.
• Purchase of a membership does not include parking privileges. Please observe campus parking regulations. Meters are available in either Panther or Parkview Garages.
• PantherFIT group fitness classes require prior registration. Online registration is open 1 hour prior to class start; call-in registration is available for WRC Gold Members.
• Memberships purchased at the BBC Wellness & Recreation Center are accepted at MMC only if the member also registers as an MMC WRC member.
• The WRC has been designated by FIU as a hurricane shelter for Monroe County (the Florida Keys), and at times may need to close to serve that role, even if emergency status has not been declared in Miami-Dade County. Such closure(s) is/are not grounds for membership refund.
• From time to time, photographs of members participating in activities may taken for departmental and/or University promotional purposes. Note that your signature on the Membership Application indicates your agreement to this. Common courtesy and discretion will be used in any photography.
• MEMBERSHIP IS A PRIVILEGE. The Wellness & Recreation Center - MMC reserves the right to terminate ANY WRC membership at any time (and without notice) for disruptive conduct, misconduct or a violation of FIU or WRC policies or rules, including the FIU Student Code of Conduct and FIU Employee Policies & Procedures.
