Introduction: As part of the Fundraising Committee’s efforts to determine the best way to approach
industries and businesses for their corporate support of Southwest Washington Symphony, we have
learned it will be helpful to establish some kind of connection between the Symphony and the
company to be contacted. It will be advantageous to find out if anyone from that company actually
plays in the Symphony. The purpose of this survey is to determine any connections that might be
helpful for fundraising efforts. Completing and returning this survey is completely voluntary.
