Attention All Parents of Student-Athletes:
The list below includes the requirements for a student to participate in Athletics (see attached forms):




Student-athlete registration form,
Kentucky High School Athletic Association (KHSAA) Physical Form*, and
Policy for Athletic Support of Fruit Sales.
Panda 200 Fee (make payable to “Notre Dame Academy”)
* The KHSAA is the governing body that oversees all high school athletics in Kentucky. In their bylaws, they mandate that every student-athlete must complete this entire form, be cleared by a doctor,
and have proof of medical insurance before being eligible to try-out for any high school sport.
The KHSAA form is mandatory for participation in athletics; no other form can be used in its
place.
Please do not confuse this form with the school medical form kept on file at the school office. When
you schedule your daughter’s physical, please take both the school medical form (gold sheet) and the
KHSAA Physical form to your doctor for completion. If your daughter plans to play a spring sport
make sure the physical is completed after May 30. The physical is good for one year from the exam
date.

Fall Sports: If your daughter is planning to try out for a fall sport (golf, cross country, soccer, or volleyball)
this form must be completed and turned in to Kim Gunning before July 10.
Winter & Spring Sports: If your daughter will not be playing a fall sport but is planning on playing a winter
or spring sport, this form must e completed and turned in to Kim Gunning before September 10.
Unsure? If your daughter is unsure if she will be participating we recommend that her forms still be turned in.
NOTE: Your daughter will not be able to participate in tryouts without completion of this form.
Mandatory Athletic Orientation Meeting: All freshman student-athletes and a parent or guardian
are required to attend the orientation meeting scheduled for Thursday August 22, 2013 at 7:00 pm in
the gym. This meeting is mandatory for any freshman interested in playing any sport at Notre Dame
Academy.

*Note that the family sports pass will be honored at all home games for the 2013-2014 school year for all NDA sports.
Each family will be given two passes and is for immediate family only. These passes exclude admission to tournaments.

Policy for Athletic Support of Fruit Sale
Each and every Notre Dame student that is a member of the Girls' Athletic Association has the
following responsibilities to uphold:

Every athlete is expected to sell a minimum of 17 boxes of fruit.
The Fruit Sale Kick-off day is October 4 upon receiving your packet of information you must
show your parents all information.
Any participating athlete failing to sell her required number of fruit boxes will be assessed
$20.00 for every box of fruit not sold. The charge will be collected December 13.
Fruit Sale money is due Friday December 13, 2013. Any student who does not return their her
sale money by the December 13, 2013, will not be eligible to take first semester exams.
Students who do not turn in their full amount of fruit money by
December 13 are ineligible to take semester exams.

All fall athletes must follow the same rules as the winter and spring athletes even though the
fruit sale starts after fall season.
All athletes/members must sign up to help unload the truck upon delivery of the fruit and
must sign up to help pass the fruit out on the weekend of December 6-8, 2013.
*It is in the best interest of everyone to pull together for this common goal. The athletes are the
ones who benefit the most. Coaches, administrators, and parents give endless hours to the
program and this is the one way for the athletes to chip in and help!
