I hereby authorise you, in respect of my membership of this Association, to make
deductions from my salary at the rate commensurate with my salary per month

I authorise my monthly deductions to be changed per month according to
changes in my salary as and when they occur.
I understand that, notwithstanding this instruction, responsibility for the payment
of my subscriptions correctly and at the proper time rests with myself.
