An Approved Test Provider Annual Report, signed by an appropriate authority, must be
received, along with this cover sheet, by the ASCLD/LAB Proficiency Program
Manager by April 1 of each calendar year.
The report will cover the period January 1 through December 31 of the preceding year.
Reports are to be emailed to pwilliams@ascld-lab.org.
General Report Content Expected:

Provide an overview of any relevant changes in operations, procedures, and/or facilities
during the calendar year.
Specific Report Content Required:
Provide the following:
 List of written complaints received from ASCLD/LAB accredited laboratories and
actions taken to address complaint (if any).
 Confirmation that an annual internal audit was conducted and that this audit covered
ASCLD/LAB test provider program requirements.
 Version of Quality Manual Currently in use:
 Confirmation that the Quality Manual contains the required elements as described in our
Test Provider Program document
By test number, provide the following information for each test issued during the calendar year:



Errors in test design or test data discovered after the release of the test.
Any corrective action plan(s) instituted, including what actions were actually taken.
Confirmation of retention of required test samples.
