Beginning in late 2010, Dr. Garabed
A. Fattal, founder of the Free Clinic,
was no longer leading the Free Clinic
and supporting wider efforts to bring
health care to the uninsured.
Dr. Fattal’s presence at the Free Clinic
is missed but his vision still guides us.
In 2011, we continued our primary
mission to provide medical care and
prescription medications. Over 900
patients were served during more than
2700 patient visits while simultaneously positioning the Free Clinic for
changes ahead. In fulfillment of our
secondary mission of medical education, a steady stream of graduate and
undergraduate medical, nursing,
pharmacy and other health care
students participated in the delivery of
services to our patients.
Federal legislation. The need for the
Free Clinic will remain as an estimated
26 million people still will not have
health insurance.
Dr. Fattal has often been asked for
his vision of the future of the Free
Clinic. Consistently he has stated that
he hopes a day will come when the
Free Clinic is not needed because
everyone in this country is receiving
quality, affordable health care. By
2014, the number of uninsured in this
country is expected to drop by 32
million people as a result of recent

subsidies to purchase health insurance,
establishment of health exchanges and
other reforms will be phased in over the
next few years. These changes in the
health care delivery system will alter the
availability of health services for those
people who were previously unable to

of these reforms. The number of our
Free Clinic patients that will benefit from
these changes is unknown.
In response to patient uncertainty
about eligibility for Medicaid and other
programs, we have implemented a
program to screen patients for
participation in these programs. The
response to screening has generally
been positive. Many of the people
screened are grateful to learn they
qualify for insurance. This assistance in
identifying health insurance options for
our patients will be important as new
options become available.

The Uninsured
Free Clinic patients are not unlike other people in this country who lack health insurance.
Commonalities in persons who are uninsured include:

About one-quarter of uninsured adults go without needed care due to cost

The uninsured often face unaffordable medical bills when they do seek care, adding
to the stress of struggling to make ends meet

Most uninsured are of low to moderate incomes – 41% below the Federal poverty
guidelines

Three-quarters of the uninsured are in working families. Often their employers either
do not offer health insurance benefits or the worker cannot afford their share of the
premium, especially for family coverage. Persistent high unemployment puts
employer-sponsored insurance at risk.

Health insurance coverage is expected to be available to over 32 million previously uninsured
people. Strategies that will be implemented between now and 2014 to help provide coverage
include:

Expansion of Medicaid to people with incomes up to 138% of the Federal poverty
guidelines

Requirement for all U.S. citizens and legal residents to have health coverage

Creation of health exchanges for people to obtain insurance when insurance is not
available through employers or they are not eligible for other health insurances

Premium and cost-sharing subsidies to purchase insurance out of the exchanges for
people who earn less than 400% of the Federal poverty guidelines.
