DATE (Date the item was assigned by the radio room)
DISPO (Disposition of the complaint, whether the deputy wrote a report, it was
sent to another shift, etc.
FOLLOW UP (The follow up is who it is assigned to, example deputy, water
patrol, narcotics, etc.
SIGNAL (What is the call for service classified as, example 21, 103, 52F)
NATURE OF COMPLAINT (What is the description of the signal, example 21speak to deputy, 103 – disturbance, 52F- fire)
DISPATCH TIME (The time the radio room dispatched the call to the deputy)
DEPUTY’S REPORT (A description of what took place from the time the deputy
was dispatched to the complaint until he completed the call)
PAGE OF PAGE (List the number of pages beginning with the first page and
ending with the last page, example 1of 2 pages)
DEPUTY NAME AND BADGE NUMBER (The printed name and badge
number of the deputy completing this report)
