STUDENT’S NAME: EWU ID#: (PLEASE PRINT) EASTERN WASHINGTON UNIVERSITY FINANCIAL AID AND SCHOLARSHIP OFFICE MAXIMUM TIME FRAME APPEAL FORM Federal financial aid eligibility for undergraduates is suspended at 250 attempted quarter-based credit hours or 165 attempted semester-based credit hours and includes all transfer credits accepted by EWU. Pre-university (remedial) credit hours do not count toward this credit limit. You may appeal your suspension, and if approved, receive an extension of aid to cover the terms needed to finish your degree. However, it is not guaranteed that you will be granted the same number of terms that you request. For your financial aid to extend past 270 attempted credits, you must have attempted and earned credits that do not count towards your degree. This determination is made based on your current program of study. Any changes to your program of study after the submission of this appeal may result in the denial of future financial aid. The approval of this appeal is contingent on you arranging appropriate plans for the completion of your degree and refraining from registering for courses that do not meet degree requirements. In order to appeal, all transcripts from your prior institutions must be submitted to the EWU’s Office of Records and Registration. In addition to the maximum time frame requirement, you must also be in good standing with all other standards detailed in the Satisfactory Academic Progress Policy, available at www.ewu.edu/financialaid. Federal regulations define separate limits on the total amount of Pell Grant students may receive and the total amount of Direct Loans students may borrow. This appeal process does not increase those limits. Instructions: Explain the circumstances that caused you to not complete your degree within the maximum time frame allotted. Type your response on a separate sheet and attach to this form. Fill out the attached degree completion plan and have it signed by your academic advisor. Deadline: Appeals must be received in our office by close of business, 5:00 p.m. on the 10th class day to be considered for the current term. Appeals submitted after the 10th day of the term will be considered for the next term. Financial aid cannot be reinstated for a prior term. By signing this appeal, I understand it is my responsibility to fully explain the circumstances that caused me to exceed the maximum time frame requirement and to adhere to the courses listed in my degree completion plan. I understand that if my appeal is denied, or if my appeal is approved and I fail to graduate in the approved time frame, I will be ineligible for further financial aid. _________________________________________ _________________ ______________________ Student Signature Date Phone Number For Financial Aid Office use only: Approved ___ Denied ___ Initials _____ Date _____________ Term _____________ Total attempted credits _______ Excludable credits _______ Terms of eligibility remaining (if approved) _______ Qtr Sem Page 1 of 2 Updated 06/23/15 DEGREE COMPLETION PLAN List the courses that you plan to take in order to earn your degree, including the current term.
