insurance plan, offered through the State Health Benefit Plan,

Fitness Programs provide advice and support for physical wellness.
Evercare™ Solutions for Caregivers provides professional support
for mental and emotional health.

NurseLineSM Services
Speak with a registered nurse, 24 hours a day, 7 days a week.
• Discuss your health and diet.
• Review or discuss your medications.
• Receive information about illnesses and injuries.
• Get tips on working with your doctor.
Wellness Advising Program
Reducing your risk for falls and managing a healthy weight are common challenges for older adults.
If you face these challenges, you may be eligible to enroll in our Wellness Advising Program. Our wellness
advisors will help you create a plan for healthy change and provide valuable tools and support to help
you reach your goals.
The products and services described below are neither offered nor guaranteed under our contract with
the Medicare program. In addition, they are not subject to the Medicare appeals process. Any disputes
regarding these products and services may be subject to the UnitedHealthcare grievance process.
Health Discount Program
The OptumHealth Allies discount program can save you and your family money on health and wellness
purchases that are not included in your standard health benefit plan. These discounts can be used for:
weight loss programs, dental care, alternative care, hearing aids, long-term care services and more.
You will receive a separate discount card to access these benefits.
Disclosure: The OptumHealth Allies discount plan is administered by HealthAllies,® Inc., a discount
medical plan organization. The OptumHealth Allies discount plan is NOT insurance.
The OptumHealth Allies discount plan provides discounts at certain health care providers for medical
services. The OptumHealth Allies discount plan does not make payments directly to the providers of
medical services. The discount plan member is obligated to pay for all health care services but will receive
a discount from those health care providers who have contracted with the discount plan organization.

a program that supports you, your family and your loved ones.
Services provided by our registered nurses include:

the person receiving care
•Caregiver coaching services for your family
•Unlimited 24-hour, toll-free phone access to an Eldercare
Specialist who will help connect you to local resources that

SilverSneakers Fitness Program
Stay physically fit and active with a SilverSneakers Fitness Program membership. This fitness program is
available to you at no additional cost.
Join SilverSneakers and take advantage of the following services:
•A basic fitness center membership at more than 10,000 participating locations.
•Access to SilverSneakers classes, cardio equipment, resistance machines, free weights and heated pools
(at certain locations). Amenities may vary at each location; not all locations offer all these amenities.
•Nationwide access to any participating fitness location (find locations at www.silversneakers.com).
•Many women-only locations, including Curves,® are available nationwide.
If the nearest participating location is 15 miles or more away from your home, you can register for the
SilverSneakers® Steps program. This is a personalized program that provides tools such as resistance
