Before completing and submitting this appeal, read all of the instructions and review the Satisfactory Academic Progress (SAP) requirements in the “Academic Progress” section of the website: sdsu.edu/financialaid. Lack of knowledge of the SAP requirements will not be grounds for submitting an appeal. If you have failed to meet Satisfactory Academic Progress (SAP) requirements, you may appeal your ineligibility for financial aid. The SAP requirements consider the quality of your academic performance and the pace at which you progress toward your degree, credential or certificate. You may use this appeal if: • you have not completed at least 75% of the units you attempted summer, fall, and spring; and/or • your grade point average (GPA) is below the minimum SAP standard; and/or • you have completed the maximum units and/or semesters for which you may receive aid for your degree. Note: Repeating a course more than once for course forgiveness that you previously completed with a D- or better, does not count toward the completion of 75%. Completion Rate or Minimum GPA Your appeal must be based on extenuating circumstances beyond your control that affected your ability to satisfactorily complete your classes. If you did not have extenuating circumstances, you may regain financial aid eligibility by using your own resources to attend and complete coursework until you meet the SAP standards. Review the following information and the “Instructions” on page 2. Extenuating Circumstances Extenuating circumstances beyond your control that may be considered with supporting documentation might include: • personal injury or illness that required extended recovery time or significant improvement; • death or illness of immediate family member; or • significant trauma in your life that impaired your emotional and/or physical health. Explanation and Documentation • In your signed letter of explanation (refer to the “Instructions” on page 2), explain the extenuating circumstances; when they occurred and how long they lasted. • If an illness or injury, provide medical documentation that includes the date(s) of the illness and treatment. Include a statement signed by your doctor, on his/her letterhead addressing the situation and your ability to return to school. If you are registered with Student Disability Services (SDS), provide a letter of support from your SDS counselor. • If a death in your immediate family, include a death certificate or obituary (include relationship to you). • If you are an EOP student, provide a letter of support from your EOP counselor. • Include police reports or attorney statements if appropriate to support your circumstances. • Indicate how you have addressed or resolved the issue(s) which resulted in your inability to meet Satisfactory Academic Progress requirements. Explain why it will not cause problems in the future. If available, include letters and/or documentation of your meeting(s) with an academic advisor and his or her recommendations of corrective action you should take. Page 1 of 4 Satisfactory Academic Progress (SAP) Appeal Maximum Units Explanation and Documentation • Submit the completed appeal form (page 3), a Financial Aid Satisfactory Academic Progress Academic Plan (page 4), and other supporting documentation. Listed below is the documentation you must submit with your appeal, depending on your degree or credential objective. • Undergraduate Degree — • A Graduation Evaluation (available 8 weeks after you apply for graduation) which you may print from your WebPortal record. • Or, a Degree Audit Report (Degree Evaluation) which you may print from your WebPortal record. • Or, a manual degree evaluation (required for this appeal if you have a double major or minor; available 8 weeks after you request it from the Office of Advising and Evaluations). • And, if required for your major, a Master Plan or Major Outline signed and approved by your academic department. • Single or Multiple Subject Teaching Credential — • Documentation detailing requirements you have not yet completed is available from the department managing your specific credential program. • Classified Graduate or Second Master’s Degree — • Official Program of Study. • If you are working on your thesis (799A or 799B), submit a letter from your thesis committee chair stating your progress and projected completion date of your thesis. • Doctorate Degree — • Letter from your program advisor stating that you are now or will be enrolled in coursework appropriate to your degree during the period covered by the appeal. Instructions 1. Develop a plan of action. We recommend you consult with an academic advisor for assistance in developing your plan. Your action plan must address how you have resolved the issue(s) which resulted in your inability to meet Satisfactory Academic Progress requirements. Explain why it will not cause problems in the future. If available, include letters and/or documentation of your meeting(s) with an academic advisor and his or her recommendations of corrective action you should take. Your future aid eligibility is at risk if you do not develop a reasonable plan to ensure you can meet Satisfactory Academic Progress requirements. 2. Submit the appeal by the deadline (refer to page 3) for the academic term for which you are appealing to receive financial aid. 3. Submit the completed appeal form (page 3), your signed letter of explanation, other letters and documentation that explain the extenuating circumstances beyond your control that affected your ability to satisfactorily complete your classes. Letters and documentation must be typed or written legibly. 4. If you did not complete at least 75% of the units you attempted or you did not maintain the minimum required GPA, your personal letter of explanation should include, but is not limited to: • the extenuating circumstances that prevented you from meeting SAP standards; • when the circumstances occurred and how long they lasted; • what has changed in your situation that would allow you to demonstrate satisfactory academic progress; and • the steps and action you are taking to ensure you will meet SAP standards and maintain satisfactory academic progress in the future. If your appeal is approved, you must meet the terms of your probation to continue receiving financial aid. 5. Submit everything related to your appeal at the same time. Incomplete information or documentation may result in a denied appeal. 6. Once submitted, your appeal will be evaluated and a decision made within 4-6 weeks.
