The pyramid is divided into seven levels of support beginning with basic life needs (like fair and competitive pay) at the lowest level of the pyramid. It evolves to higher level wants and desires (such as leisure, fitness, spiritual enrichment, and career advancement) at the peak of the pyramid. Sacred Heart substantially invests in Wellness University (Wellness U), a program that is designed to help you chart a personal course toward improving and maintaining personal health. Wellness U offers free, convenient classes that focus on topics such as weight loss, stress management and cardiovascular health. Sacred Heart also provides programs that support family needs such as dependent coverage in many benefit programs, and where possible, depending upon the operational needs of the department, flexible scheduling and personal leave. Another vital part of promoting well-being is our Employee Assistance Program (EAP). Through our counseling and referral system, as well as comprehensive mental health features in our medical insurance plan, we can help Associates and their family members address many of the challenges encountered with everyday living. SHHS 14 Premium Pay For certain positions, SHHS pays market-based shift differentials and on-call pay. Overtime is paid in accordance with the Fair Labor Standards Act. SHHS 15 Special Pay Where indicated by market conditions or other operating needs, SHHS provides special pay such as Referral and/or Sign- on Bonuses to ensure adequate staffing and operations. SHHS 15 Paid Time Off (PTO) Cash-In To add to regular earnings, Associates may cash-in accrued PTO in 12-hour increments at 85% of their base rate. Associates are required to maintain a balance of 40 hours after cash-in. The annual cash-in limit is 240 hours per year. SHHS 15 Paid Time Off (PTO) Sell* To help pay for benefit contributions and ultimately increase their take home earnings, Associates may “sell” up to 40 hours of accrued PTO a year at 100% of base pay during the annual open enrollment period. SHHS 15 Associate Recognition Award This program recognizes that we are all interdependent upon each other as we strive to achieve our Mission and goals. It encourages good stewardship of resources and service to our patients by providing a monetary award to Associates if Sacred Heart’s service and financial goals are achieved. SHHS 16 Outstanding Associate Award This award was developed to recognize the extraordinary contribution of non-management staff members who exemplify the Core Values and who consistently exceed their job expectations. SHHS 16 Servant Leader Award This award was established to honor those leaders who demonstrate the qualities of leadership as exemplified by the Founders of the Daughters of Charity. SHHS 16 Service Awards Associates celebrating years of service in 5-year increments are recognized by SHHS for their loyalty and commitment to our ministry. SHHS 16 Retirement Recognition Associates retiring at age 55 and older and after completing 20 years of continuous service at SHHS will be officially recognized through a reception in their honor. Available to FT Associates who meet guidelines set forth in the application. SHHS 18 Dental Insurance* Comprehensive PPO Plan with a choice of 2 coverage options: Premium Plan or Standard Plan. Benefits begin on the 15th consecutive calendar day of absence due to illness or injury. Associate premiums, if applicable, are pre-tax. Available to both PT and FT Associates. Eligible Associates may supplement their disability income by cashing in PTO at 100%. Cash-in is based on assigned hours at time of disability. SHHS pays for 50% of pay option for FT Associates, who may elect to purchase 10% or 20% additional coverage. PT Associates may purchase either 50%, 60%, or the 70% option. 26 Long-Term Disability* Benefits begin on the 91st consecutive calendar day of absence due to illness. Associate premiums, if applicable, are pre-tax. Available to both PT and FT Associates. SHHS pays for 60% of pay option for FT Associates, who may elect to purchase 10% additional coverage. PT Associates may purchase either the 60% or the 70% option. 27 Social Security & Medicare Disability benefits based on Social Security regulations. SHHS and Associate share the responsibility for funding this benefit. 27 Bereavement Pay Up to 3 days salary continuation for Associates who experience a loss of an immediate family member. Available to FT and PT Associates. SHHS 28 Jury Pay Salary continuation for Associate to uphold their civic duty while serving as a juror. SHHS pays the regular base salary less any monies received for jury duty. 28 Military Reservist Pay Salary continuation for Associates who participate in mandatory Reserve or National Guard Summer Camp. SHHS pays the regular base salary less the base military pay received. 28 Workers’ Compensation Automatic coverage in accordance with Florida state laws. Available to all Associates. SHHS 28 Malpractice and Professional Liability Insurance Coverage available to all Associates. SHHS 28 Property Insurance Personal property insurance is offered to PT and FT Associates. Coverage options include auto and boat insurance. Premiums are post-tax. Payroll deduction is available. This is an optional benefit. The Associate pays the cost of coverage. 28 Long-Term Care Insurance Comprehensive plan for nursing home and related long-term care needs. SHHS 30 Hyatt Legal Plan The Hyatt Group Legal Plan provides members with access to a national network of more than 9,500 attorneys to assist with Wills and Estate Planning, Identity Theft Defense, etc. Associate 30 Who Pays Page LEVEL III: BUILDING & MAXIMIZING FUTURE SECURITY Tax Savings Ascension Health Retirement Well-Being Program Plan Description Ascension Health Retirement Well-Being Program This program encourages Associates to save for their retirement or other long-term needs on a pre-tax basis. The Sacred Heart program has three components: Automatic Employer-Provided Benefit, 403(b) Retirement Savings Plan and 403(b) Employer Matching Contribution. SHHS and Associates share in the responsibility for funding this benefit. 32 Automatic Employer-Provided Benefit Sacred Heart offers an employer-provided benefit whether or not you make a before-tax contribution. Associates hired prior to January 1st, 2006 have a Defined Benefit Pension Plan and Associates hired on or after January 1st, 2006 have a Defined Contribution Plan. Both plans have a five year vesting requirement. SHHS 32 403(b) Retirement Savings Plan Sacred Heart Associates are eligible to participate in the Ascension Health Retirement Savings Plan. You are eligible to make contributions your first day of employment. Associate 33 403(b) Employer Matching Contribution (also referred to as 401(a) Account) If you make a contribution to the 403(b) Savings Plan, you will receive matching contributions from Sacred Heart. These matching contributions will begin the first pay period after you have worked 12 consecutive months with Sacred Heart. SHHS 33 Social Security Retirement benefits based on Social Security regulations. SHHS and Associates share in the responsibility for funding this benefit. 33 Health Care Flexible Spending Account (HFSA)* Convenient and simple way to save on taxes for certain out-ofpocket healthcare expenses such as qualified medical, dental and vision expenses not covered by Sacred Heart Insurance.
