1. Details
1.1 Setting out, excavation, formation of foundations and slab
2. Sequence of operations:
2.1 Site induction undertaken by WMD
2.2 Setting out engineer will set out grid lines and levels under supervision of the site
supervisor
2.3 The area of the works will be segregated with signage and heras fencing to
create a working zone
2.4 Excavation of the foundation / slab will be carried out by machine excavators to
the required depth not, 300mm
2.5 Formwork will be constructed to the beams and slab edges using ply and hand
held tools
2.6 The steel reinforcement will be delivered by flatbed lorry and mechanically lifted
into position by the use of lifting chains and under supervision of the banksman
2.7 The steel will be tied by hand and checked for accuracy by our engineer
2.8 Mushroom caps will be placed over exposed steel
2.9 A concrete pump permit will be completed and signed
2.10
The pump lorry will reverse under supervision of the banksman to the works
area and set up in accordance with the manufacturer’s instructions
2.11
The outriggers of the pump will be supported on railway sleepers 1.2m long x
1.2m wide
2.12
The pump operator will move the boom into position with the operative
located at the end to move the pipe to the desired location
2.13
The foundations / slab will be poured, all exposed steels will have mushroom
caps
2.14
The formwork to the foundations / slab will be struck after is has cured in
accordance with the engineers guidelines
2.15
A concrete washout area will be agreed and constructed
2.16
A jet wash will be available at the site entrance for wheel washing
2.17
The pump will be cleaned out by the use of a pig and ball, the ball is put on
the end of the hose and sucked back by vacuum into the recess at the rear of the
shute.
3. Duration:
3.1 In accordance with the master programme
4. Preparation:
4.1 A CAT and genie survey of the area will be carried out prior to starting and hand
digging
4.2 The service avoidance plan will be available
5. Access:
5.1 Access to the site is via the main entrance

6. Permit to dig:
6.1 A permit to dig is required
6.2 A permit to pump concrete is required
6.3 The site supervisor will enforce the permit
6.4 Completed permits will be retained at the site office and copies will be forwarded
to the head office by WDC
7. Supervision & Operatives:
7.1 The site supervisor will supervise all excavations
7.2 The Site supervisor holds a current SSSTS certificate
7.3 Supervisor plus 6 operatives including banksman and pump operator
8. Training:
8.1 All operatives hold a current CSCS or CPCS cards and will have them on them at
all times
9. Plant:
9.1 Excavator and concrete pump – test certificate will be made available by the
driver
9.2 Roller – test certificate will be made available by the driver
9.3 Dumpers – test certificate will be made available by the drive
10. PPE:

Hard hat, Hi-viz, safety boots and gloves must be worn at all times
Wellington boots will be used if in contact with concrete
Key fobs will be used by all plant operators

Manual handling limit of 20kg per operative will be in operation
12. Waste:
12.1
All waste will be treated in accordance with the site waste management plan
13. Control measures:
13.1
Fire extinguishers are located as per the fire plan
13.2
In the event of an emergency the site supervisor will contact the emergency
services in accordance with WDC emergency plan
