This is a new feature containing stories that show how individuals’
lives are positively changed by using services provided at New Horizons ILC. *Names have been changed to protect their privacy.
The VITA free tax assistance program is very helpful
to our consumers. Several individuals have been to the
Prescott Valley location which has good access. Some
people had requested peer mentoring assistance and are
very happy to have already received their tax refunds.

Section 5317 Transportation Program
The Section 5317 New Freedom Transportation Program provides transportation to persons with disabilities to address needs that go beyond those required by the ADA. This funding provides transportation to
medical appointments, grocery shopping and to meet other needs and requests for rides. Call New Horizons
Transportation at 775-8870 to ask for Section 5317 transportation rides. There is limited funding available,
so call soon for information.
Free Tax Preparation:
VITA (Volunteer Income Tax Assistance) is available for low income households. Call 211 or 1-877-2118661 or go to www.211arizona.org Bring, Social Security card, photo ID, W2s and 1099 forms, ID for
all family members, childcare receipts, #s of childcare provider, checking account information for direct
deposit, and copy of 2011 tax return.

Prescott: Catholic Charities, 434 W. Gurley St., Saturdays 9-4, Wednesdays 5-8pm. Closed March 30.
Prescott: Yavapai College, 1100 E. Sheldon, Bldg 3, Room 204, Fridays 10:30-3:30. Closed March 29.
Prescott Valley: Catholic Charities, 3044 Tani Rd. on Saturdays 9-4 and Thursdays 5-8 pm. Closed March
30. Cottonwood: Catholic Charities, 736 N. Main St., Saturdays 9-4, Tuesdays 5-8 pm. Closed March 30.
Career Exploration & Consultation: will be held Tuesday, April 16 from noon-4:00 p.m. at Catholic
Charities Conference Room, 434 W. Gurley, Prescott. This class will include Career Assessment Testing
plus assessment results and personalized consultation. Space is limited to 15. Register by Friday, April 12 by
calling Milissa at 708-7223 to reserve your spot.
Caregiver Support Groups by Area Agency on Aging:
*Held at Mayer Area Activity Center, 10051 Miami Street on Tuesdays from 12:45-2:00 p.m. - starts on
April 9 for 8 weeks.
*Held at Susan J Rheem Adult Day Centers (SJRC) in Prescott and Prescott Valley: every other Thursday
from 3:00-4:00 p.m. Care for your loved one is provided by SJRC while you attend the support group at no
charge. Please call ahead to get information.
Starts April 4 in Prescott, 826 Sunset Ave. Call Karen Brewer, Director Prescott SJRC at 445-6384.
Starts April 11 in Prescott Valley, 3407 N. Windsong Dr. Call Laura Dreibelbis, Director Prescott Valley

Cost is free. Support groups are open to anyone providing care for another adult. For more information and

Family Caregiver Classes: on May 20 & 21, 2013 at The Margaret T. Morris Center, 878 Sunset Ave.,
Prescott from 9:00 a.m.-noon. No cost. Continental breakfast provided. Care for your loved one will be
provided while you attend the training. Participants must register for classes and for respite services by May
6 and must attend both classes. Call 1-877-521-3500.

MCS Walkathon Across America: sponsored by the Jennifer Parker Foundation, is taking place in many
cities across the U.S. on Sunday, May 5, 2013 to raise awareness of Multiple Chemical Sensitivity (MCS)
and Environmental Illness. The local walk will be at Fain Park, 2200 N. 5th Street in Prescott Valley. Walk

Arizona Caregiver Resource Line is a new statewide toll-free line and website for resources for family
caregivers. Go to www.azcaregiver.org or call the Caregiver Resource Line at 1-888-737-7494 or Email to
CRL@azcaregiver.net. Leave a message anytime. Volunteers will call you Monday-Friday from 9 am-4 pm.
This service is an easy way to get information related for caregivers.

NHILC Arts and Crafts Group - Creative Crafters, meets the first Tuesday each month at 2:00 p.m. The
next dates are April 2 and May 7. Class includes materials and instructions.

Dave’s Out to Lunch Bunch meets on the 2nd Thursday each month at noon. The next date is April 11

MCS (Multiple Chemical Sensitivities) Support Group meets on the 3rd Tuesday at 2 p.m. The next
dates are March 19 and April 16.
Chronic Fatigue Immune Dysfunction Syndrome (CFIDS) and Fibromyalgia Support Group meets
on the 3rd Thursday at 2 p.m. at NHILC. The next dates are March 21 and April 18.
Brain Injury Support Group (B.I.G.) meets on the 4th Thursday from 2 to 3 p.m. each month. The next
dates are March 28 and April 25. Call Mary at 772-1266.
