Healthy Indiana Plan: Important Information Before 2015, there was only one HIP plan. Starting in 2015, there will be more plans with different benefits. Plan Changes: • HIP Plus • Preferred plan with the best value • Meant to cover everyone with household income below 138% of the Federal Poverty Level (FPL) • $1,358 a month for one person or $1,831 a month for a family of four • Vision and dental benefits • One low monthly POWER Account contribution • After your contribution, you will not owe anything when you get medical services (There will be a copay for going to the ER when it is not an emergency) • HIP Basic • Only for members with income less than 100% of the FPL who do not make their monthly contribution • $973 a month for one person or $1,988 a month for a family of four • There are no dental or vision services • No monthly POWER Account contribution • Payments between $4 and $75 are required for most health services • HIP Basic could be more expensive than paying your small monthly HIP Plus POWER Account contributions HIP2M0044 (1/15) 1 • HIP State Plan Plus • HIP State Plan gives you a different set of benefits that work best for your situation or medical condition for a low, predictable monthly cost • Meant to cover everyone with a household income below 138% of the Federal Poverty Level (FPL) • $1,358 a month for one person or $1,831 a month for a family of four • Vision and dental benefits • One low monthly POWER Account contribution • After your contribution, you will not owe anything when you get medical services (there will be a copay for going to the ER when it is not an emergency) • For more information about HIP State Plans visit MDwise.org or call MDwise customer service at 1-800-356-1204 • HIP State Plan Basic • For members with income less than 100% of the FPL who do not make their monthly contribution on time • $973 a month for one person or $1,988 a month for a family of four • Vision and dental benefits • No monthly POWER Account contribution • Payments between $4 and $75 are required for most health services • HIP State Plan Basic may be more expensive than paying your small monthly HIP State Plan Plus POWER Account contributions 2 POWER Account Changes: • POWER Accounts will increase from $1,100 to $2,500 • You will pay a small monthly contribution into this account • The State will pay the rest • This money will go towards paying for your covered medical services during the year • If you do not use all of the money in your POWER Account, your contributions can be smaller next year • If you leave the program early you may still have to pay your portion of your POWER Account • If you get the preventive services, your contributions can be smaller next year • If your spouse is on HIP as well, you will be able to split your contribution evenly • Example: instead of owing $10, you each owe $5 Pregnant HIP Members: • Whatever plan you are on, you can keep it • If your benefit plan year ends while you are pregnant, you will be moved to HIP Maternity Plan for the remainder of your pregnancy • The cost for medical services won’t be deducted from your POWER Account • You will not owe a POWER Account contribution or co-pays for medical services • You will have vision, dental, and transportation benefits 3
