Submit Student ID REQUEST FOR BOOK ADVANCE DISBURSEMENT USING LSCS CAMPUS BOOKSTORE I, , (“Student”) am currently enrolled and in good standing as a College Student at Lone Star College. I am submitting my book advance request to the LSC- Campus/Center. I have applied for and have been awarded Federal Student Financial Aid in the form of loans and/or grants to be used for the payment of education expenses. Further, I (1) am registered for classes with intent to attend, (2) meet academic standard policies, and (3) am not in default on any current student loans. I understand that I am not under any obligation to purchase textbooks and supplies from a particular bookstore or vendor and that textbooks and supplies may be available from any campus or off campus bookstores and/or online retailers. As long as I am a financial aid recipient, I desire that an amount not to exceed $ 1,000.00 of my Student Financial Aid be made available with Follett Bookstores, at all Lone Star College campuses/centers, in purchasing textbooks for the duration of my enrollment at Lone Star College. If I decide to purchase books from a non-LSCS affiliated off campus book store during future terms, I will notify the Financial Aid Department. Should any of the disclosures be incorrect; or should I withdraw as a Student for the College Semester after I am in receipt of the Book Advance Disbursement, I acknowledge and consent to the College taking all actions as allowed by applicable law(s) and regulations in collecting the Book Advance Disbursement.
