Please make checks payable to Carthage Chamber of Commerce with “Maple Leaf” in the memo line. Each booth will
rent for the above listed price if payment is received by September 1, 2011. For payments received after September 1st,
there will be a $10 late fee. Limited access to electricity may be available for booths, but must be noted on the
application. Refund for booth space will be given (less a $10 jury fee and a $10 processing fee) until October 1, 2011.
No refunds will be given after October 1, 2011.
By submitting the application to the Maple Leaf Festival Committee, the undersigned acknowledges receiving, reading and fully
understanding all of the guidelines and regulations of the 2011 Maple Leaf Festival Committee. I understand that submission of this
application with the required fees and documentation does not guarantee my admission to the festival as a vendor. I further
understand that the $10 jury fee and $10 processing fee enclosed are non-refundable. The Maple Leaf Festival is sponsored by the
Carthage Chamber of Commerce. The Carthage Chamber of Commerce is not responsible for accidents or theft. If you have any

Hold Harmless and Indemnify By Vendor Applicant
Vendor and his/her representatives agree to indemnify, defend and hold harmless the Carthage Chamber of Commerce, Carthage
Convention & Visitors Bureau, City of Carthage, County of Jasper, their officials, members, agents, employees and representatives
from and against any and all claims, demands, defense costs, liability or consequential damages of any kind or nature (hereinafter
referred to as “Claims”) arising out of or in connection with Vendor’s work, performance or failure to perform, under the terms of
this contract; except for those Claims which arise out of the sole negligence or willful misconduct of the Carthage Chamber of
Commerce, Carthage Convention & Visitors Bureau, the City of Carthage, and/or the County of Jasper.

2011 MAPLE LEAF FESTIVAL
CRAFT and CONCESSION BOOTH
GUIDELINES & REGULATIONS
1. Any craft vendor seeking a booth in this festival must submit the attached application together with all of the
following:
a. Proof of liability insurance coverage.
b. Booth Rental Fee. (Fee includes a NONREFUNDABLE Jury Fee of $10 and Processing Fee of $10.)
c. Photograph(s) of every item to be sold in the vendor’s booth. The photographs will be used to
determine the acceptability of each proposed craft for this festival; therefore, all items must be clearly
visible and recognizable in the photographs. Any item not clearly represented in the application photo(s)
may be excluded or rejected from the festival by the Maple Leaf Committee at any time, without
question. All photographs must have the name, address and phone number on the back of each
photograph. Photographs will not be returned.
2. Any concession vendor seeking a booth in this festival must submit the attached application together with all of
the following:
a. Proof of liability insurance coverage.
b. Booth Rental Fee. (Fee includes a NONREFUNDABLE Processing Fee of $10.)
c. Full menu of items to be sold and photograph(s) of booth set-up. The photographs will be used to
determine the acceptability of each vendor’s booth; therefore, photos must be clear and recognizable.
Any menu item may be rejected, if that food category is already full. Any menu item not listed on the
application may be rejected by the Maple Leaf Committee at any time, without question. All
photographs must have the name, address and phone number on the back of each photograph.
Photographs will not be returned.
3. All crafts must be hand-made. Pre-made, store bought, or market items may not be sold or displayed in this
festival unless significant ornamentation, hand-made by the booth vendor, is added. All items to be sold or
displayed at this festival must be approved by the Maple Leaf jury committee and must show significant work by
the crafter.
a. Screen-printed items may not be sold or displayed in this festival.
b. Iron-on appliqués or transfers may only be accepted if hand applied by the booth vendor and used as a
base for significant ornamentation by the crafter.
c. Home canned or jarred items are not permitted, due to Jasper County Health Department regulations.
d. All floral arrangements to be sold or displayed at this festival must be designed and assembled by hand
by the booth vendor. Pre-made bushes or arrangements, such as those that come from market, will not
be accepted at this festival.
4. NO commercially produced products, imported products, or products sold through home sales may be displayed
or sold at this venue. Examples of prohibited product lines include: Mary Kay, Avon, Longaberger, Party Lite,
Pampered Chef, Home Interiors, Scentsy Candles, and Creative Memories. The above list is not exhaustive.
5. All vendors should plan to have their booth open and staffed from 8 a.m. until 5 p.m.
6. Requests for specific booth assignments will be granted to returning vendors, if possible, only if your completed
application packet (including payment) is received before July 1.
7. YOU MUST CHARGE AND PAY SALES TAX. A tax table and form for mailing your taxes to the State will be
included in your vendor packet. We are required to provide a full list of all vendors to the Missouri Dept of
Revenue, and
those not reporting will be contacted directly by them.
8. All craft vendors must participate in the jury process to be considered for a booth space at this festival. The
Maple Leaf Festival Committee will appoint the jury committee and any decision of the jury is final. Concession
vendors are excluded from the jury process.
9. Refund for booth space will be given less a $10 processing fee (for all vendors) and less a $10 jury fee (for craft
vendors) until October 1, 2011. No refunds will be given after October 1, 2011.
10. If you use a tent, it must be free standing and fit within your booth space and you may not use ground poles.
Some booth spaces may have light poles or other permanent structures. We cannot and will not rearrange
booth assignments because your tent doesn’t fit into the designated space.
11. Concession vendors must refer to the enclosed information with regard to available power and connectability.
It is the vendor’s responsibility to assure that his equipment is compatible or arrange for their own adaptors.
110-watt 20 amp and 220-watt 50 amp receptacles are available to concession booths on a first-come, firstserved basis. Please note that 220-watt receptacles are very limited, so vendors will need to plan accordingly.
No animals are allowed in any booth or in the downtown booth area, unless they are service animals.
