2013 American Girl Fashion Show
November 16 & 17

Join the fun on stage during intermission as a guest model! For an additional $25 donation to the Junior
League of Pittsburgh’s Healthy Kids Initiative, your American girl and her doll will walk the runway,
wearing an American Girl Fashion Show souvenir T-shirt that is hers to keep. Each guest model will be
introduced as she takes her turn on the catwalk. Girls must be in grades 1 – 6 in order to participate.
Space is limited to 10 per show and available on a first-come, first-serve basis. Please buy fashion show
tickets and guest model spots online at www.jlpgh.org. Each guest model must also purchase a fashion
show ticket to attend the event. Once tickets are purchased, please send in guest model application.
Guest model applications will be accepted until October 31, 2013.

I give my daughter permission to be a guest model in the American Girl Fashion Show. I also understand
that my daughter is guaranteed a guest model position only with a confirmed purchase of the Guest
Model spot when purchasing tickets to the show.
I understand the commitment that I am making and give permission for my daughter to participate in the
American Girl Fashion Show as a guest model. I understand that if I have any questions regarding these
matters, it is my responsibility to ask for answers before signing permission. By signing this application, I
am also giving permission for my child to be photographed and/or videotaped. These images will be the
property of the Junior League of Pittsburgh and I grant permission to the Junior League of Pittsburgh to
use photos and videos to promote the Junior League of Pittsburgh.
I agree to release and hereby release and hold harmless The Junior League of Pittsburgh, American Girl
Programs, Inc., their parent, affiliates, subsidiaries, officers, employees, agents, and assignees; and any
other sponsors, volunteers, and staff for the event listed above, from any and all liability that may arise
from my daughter’s participation in the meetings, fittings, rehearsals, or fashion show described above.
