VEHICULAR ACCIDENTS 1. Calapan City, Oriental Mindoro • On or about 03:30 AM, 13 April 2011, a vehicular accident involving a Ceres Transport Bus occurred along the Strong Republic Nautical Highway, Sitio Calauan, Brgy. Lumangbayan, Calapan City • Said bus, coming from Roxas, Oriental Mindoro bound for Calapan City Pier with 39 passengers, failed to negotiate a right turn/curve while traversing the highway because one of the rear double tires on the left side of the bus was flat. Hence, the bus veered out of control and hit a gravel stockpile causing it to fall on its left side • The driver, identified as Darlie Dave Lim y Ampan, 35 years old. married and a resident of Telecom Compound, Tampoy II, Valenzuela City, initially escaped but voluntarily surrendered afterwards to the Calapan City Police Station Casualties • There were 19 passengers injured, four (4) of whom are still confined at Maria Estrella General Hospital in Brgy. Tawiran, Calapan City while the 15 were already discharged as they only suffered minor injuries. Tabuk City, Kalinga • On or about 08:45 AM 14 April 2011, a vehicular accident involving a passenger jeep bound for Bulanao, Tabuk City, Kalinga accidentally fell down on a 30m ravine along Sitio FF Bulanao • There were 17 passengers injured identified as follows: Datul, Martin Jr, 11 years old; Gaa-yon, Flora, 69 years old; Gatan, Chery Mar, 17 years old; Aowing, Venus, 18 years old; Calis, Armor, 12 years old; Oggas, Milagros, 23 years old; Calis, Shyne, 22 years old; Tongdo, Joy, 25 years old; Calis, Joel, 13 years old; Siagan, Aspin, 8 years old; Lapangan, Parson, 35 years old; Siagan, Emerald, 1 year old; Siagan, Criselda, 28 years old; Miguel, Cristina, 47 years old; Ansiano, Casinta, 35 years old; Calis, Pearl, 9 years old; and Agcaoili, Cesar, 52 years old Actions Taken: • Victims were rushed to Kalinga Provincial Hospital, one of them was admitted, others were treated for minor injuries B. FIRE INCIDENTS 1. Cagayan de Oro City (Final Report) • On or about 3:15 AM 12 April 2011, a fire of still undetermined cause occurred at Velez Street, Cagayan de Oro City. The fire was put out at around 7:44 PM • The following structure such as: the Misamis Oriental General Comprehensive High School; 22 classrooms of Atty. Aquilino Pimentel, Sr. High School; an automotive shop, and the North Grandstand of the Pelaez Sports Center were burned • The estimated cost of damages to properties was placed at PhP1,000,000.00 2. Tondo, Manila • On April 12, 2011, a fire of still undetermined cause occurred at the residential houses in Tao-lino Street, Balut, Tondo, Manila. The fire was placed under control at around 10:46 AM and was put out at 11:03 PM • There was no casualty reported and the estimated cost of damages has yet to be determined Actions Taken: • Tondo, Gagalangin and San Nicolas Fire Stations (FSTN) have responded to the incident • OCD- X is continuously monitoring the situation to determine the extent of damages C. EARTHQUAKE INFORMATION NO. 1 • An earthquake incident of tectonic origin occurred on or about 15 April 2011 at 12:21P.M., located at 09.50ºN, 126.79ºE – 77 kms S 65ºE of General Luna (Siargao). Depth of Focus (km) - 001 (Shallow) with magnitude 3.6 (Ms) and with Intensity 1 at Surigao City; General Luna, Surigao del Norte. Actions Taken: Disseminated said earthquake information thru fax and SMS to OCDRC CARAGA and other NDRRMC key officials. 2
