Liability Waiver and Use Agreement
I, hereby assume all responsibility for, and the risks and hazards of, participation in the rental activity planned by my group in
consideration of the City of West Liberty providing permission to use the facility/facilities requested for the fee of $

rental hours, I, and all the members of
(organization), do hereby release
the City of West Liberty Parks and Recreation Department of any and all claims, demands, rights, and causes of action of whatever
kind and nature, arising from and by reason of, and all known and unknown, foreseen and unforeseen, bodily and personal injuries,
damage to property, and the consequences thereof, resulting from participation in the rental activity planned on the City of West
Liberty facilities.

Field Rental Guidelines
 To access the field reservation schedule please visit http://wlparksandrec.org/article/practiceschedules
 Athletic fields are available at both Dutton Sports Complex and Wapsi Park
 City sponsored events have priority of use
 The season registration window is March 22nd – October 31st
 The daily field reservation window is 8 am – 10 pm
 Games and tournaments can be scheduled throughout the season
 Practices can be scheduled a maximum of two weeks in advance
 Fields can be reserved for a minimum of one hour
 Practice reservations will be limited to 1 ½ hours
 Game reservations will be limited to:
o 2 hours for 1 game
o 3 hours for a doubleheader
o ≥4 hours will be charged the daily rate
 Hourly rate reservations must be requested at least 24 hours in advance to be considered
 Daily rate and weekend (Saturday and Sunday) reservations must be requested at least one
week in advance to be considered
 Reservations will not be finalized until full payment is received
 WLPR will have final say on whether the field will be playable in inclement weather situations
 Rainouts will be rescheduled, no refunds
 Lining/chalking field is a one-time set up per field per day
 Field renters must be at least 18 years of age
 Person who rented the field must be on-site when the field is being used
 Any reservation for Wapsi North and/or South diamond(s) after 8 pm will include fee for the
use of lights
 It is the permit holder’s responsibility to turn lights on and off if being used

 Copies can be turned into City Hall (409 N Calhoun St.) Monday – Friday between the hours
