Looking at a picture, we can guess at its context, the relationships between the people in the photo, and what their lives were like. We would like to share with you the stories of some of the people Benjamin Rose helps, to put a face on those we serve. These people personify Benjamin Rose’s mission: To advance the health, independence, and dignity of older adults by raising the standards for quality care. 2003 was a year in which we carefully studied our own organizational picture. We see ourselves continuing to pursue the vision Mr. Rose set forth 95 years ago. However, the Benjamin Rose of today finds itself in a vastly different climate than the Benjamin Rose of 1908. There is a burgeoning older population, often with intense and diverse health and social needs, and a looming crisis in public funding from Medicare, Medicaid, and other sources. We took on the challenge of setting some priorities for our future. The Planning Committee of the Board of Directors, its Strategic Plan Steering Group (which consisted of over 80 representatives from the community), and the executive management team met 20 times over a six-month period to develop a three-year plan. The result is our Strategic Plan for 2003-2006. A key component of the plan calls for us to begin to advocate more aggressively on behalf of older adults. Advocacy is a natural extension for an historically innovative organization that provides services and conducts research. In the coming years, we will set into motion efforts to coordinate our practice and research activities to develop recommendations for changes in public policy as it relates to seniors. We have also reaffirmed our commitment to primarily serving those of limited resources. To show you—our supporters, colleagues, and friends in the community—how we are continuing to meet our mission while planning for the future, we are including in this report several “snapshots” of Benjamin Rose’s story as an organization. We hope you, despite the ever-changing big picture around us, will join us and work with us to keep our clients and those who care for them our main focus. Catherine D. LoPresti, Chairman, Board of Directors Richard Browdie, President/CEO The Benjamin Rose Story: 95 Years of Service Early recipients (circa 1915) of care from Benjamin Rose. Benjamin Rose Hospital (19531969) was one of the first facilities in the nation designed to prepare older people to go back into their homes after surgery or illness. Today, this care is provided at Kethley House at Benjamin Rose Place. A resident of Belford House, one of Benjamin Rose’s first residential homes, reads to children in the 1940s. In 1996, the last beam of what is to become Kethley House at Benjamin Rose Place is put in place. Applied research became a part of Benjamin Rose’s operations in 1961. Today the Margaret Blenkner Research Institute is internationally renowned. In-home care today is carried out with the dignity Mr. Rose spoke of in 1908. 1 In 2003, Benjamin Rose Community/In-Home Services served 1,318 people throughout Cuyahoga County through the programs below. Some clients received care from more than one of the following programs: Adult Day/Partial Hospitalization Program Benjamin Rose Home Care (private pay program) Health Services Mental Health Program Social Services Throughout the year, staff from Community/In-Home Services held a number of caregiver support groups and a caregiver education workshop funded in part by the Western Reserve Area Agency on Aging. Caregivers learned about living wills, Medicaid spend-down, and durable powers of attorney from an elder law attorney. The workshop also offered lunch, stress reduction techniques (including on-site massage therapy), and resources to help ease the task of caregiving. * * * Benjamin Rose is the fiscal agent and a major supporter of The Greater Cleveland Elder Abuse/ Domestic Violence Roundtable. Made up of representatives from over 40 organizations, the Roundtable provides community education to address the problems of elder abuse and domestic violence in late life. 2 In 2003, the Roundtable educated 289 members of faith communities, including clergy and parish nurses. The Safe at Home presentations * * * Sylvia Galizio and Alberta Chokshi are social workers who handle queries from over 250 callers per month. Their work is part of Benjamin Rose’s information and referral consultation services. Most calls to 216.791.8000 are from concerned family members of an older adult and other service providers in the community. Each call takes an average of 15-30 minutes to answer and provide information. Approximately 25% of the calls become Benjamin Rose cases. Educating the community. were well-received, and 93% of the attendees said they would seek help if they encountered a situation of elder abuse or domestic violence in late life. The project was funded by a Violence Against Women Act grant. Calling Benjamin Rose. Rita Gray, 72, in-home client, Community/In-Home Services Rita’s Story: “ I had a happy childhood in Pennsylvania. My dad made concrete; my mother raised seven children. I was the third oldest. I received a two-year teaching certificate in Pittsburgh. I went into teaching because I’ve always loved children. I receive a lot of different services from Benjamin Rose. I have a Senior Companion Mondays and Tuesdays for four hours, a home care aide Wednesdays, the Day Program Thursdays, and on Fridays another Senior Companion takes me grocery shopping. My nurse comes once every two weeks, and my social worker comes when I ask her to stop by. My Companion, Warnell Ruffin, has been with me for eight years. We have peaceful times together. We watch TV—Golden Girls, Mad About You, The Nanny. We exercise, too; we do tai chi and Richard Simmons. Warnell also helps me with housework, goes with me to doctors’ appointments, and helps me run errands. Practicing tai chi with Warnell. My husband and I moved to Cleveland. I taught second grade, and together he and I had six children. We’re divorced now, but I see my children and grandchildren every week, unless somebody’s sick or can’t make it. I just visited my brother and sister-in-law in Pennsylvania two weeks ago. I’ve lived at Abington Arms for 10 years. I’ve had problems with depression for almost as long. My social worker at Abington Arms, Carol Ferstman, suggested I go to Benjamin Rose’s Adult Day/Partial Hospitalization Program. Sometimes we make therapeutic collages; sometimes we talk about books. I’m glad I have Benjamin Rose and Warnell. This building is good for me as well. It’s clean, and the manager always has activities planned. ” Making therapeutic collages. 3 Kethley House at Benjamin Rose Place is a 184-bed skilled nursing facility that offers short-term rehabilitative and long-term residential services. Specific care areas include: sub-acute, skilled nursing, intermediate, dementia, hospice, and respite. In 2003, Kethley House at Benjamin Rose Place conducted its sixth Nurse Aide Training and Competency Evaluation Program (NATCEP). Kethley House began participating in the state-certified program in 2001 to train, hire, and retain nurse assistants to meet Kethley House’s high standards in providing quality care. Since then, 40 out of 51 candidates have successfully completed the program. Kethley House hired 35 out of the 40 nurse assistants, and still employs 22, which is a 63% retention rate. In August, Kethley House finished its conversion to an electronic medical record (EMR) system. The EMR system uses wireless touch-screen technology to improve the quality of documentation of resident information. Resident care is ensured because nurses save time in completing necessary forms, reduce duplication, and can administer medication more efficiently. Kethley House is one of the first longterm care facilities in the Greater Cleveland area to implement this type of technology. * * * * * * Kethley House continues to rank consistently high in its annual survey conducted by the Ohio Department of Health. Longterm care facilities are examined by state surveyors to ensure that the provider is in compliance with Medicare/Medicaid requirements as well as state law. The surveyors check building operations and Taking the NATCEP exam. 4 maintenance as well as all aspects of resident care, including the admission process, nursing, activities, and housekeeping. Preparing for state survey. Kethley House was home to 613 older adults in 2003, and offered long-term care, shortterm rehabilitation, and hospice services to these individuals. James Peterson, 79, resident, Kethley House at Benjamin Rose Place Enjoying a visit from family. James’ Story: “ I was born in 1924 in Clayton, Alabama. I’ve been deaf and mute since birth. I moved to Cleveland in 1947, and lived near E. 93rd Street. In 1959, I married my wife, Dorothy. I worked as a janitor for the Lamb Baking Company until I retired in 1989. Dorothy died in 1993. I came to Kethley House in December 2001. I started out in the short-term rehabilitation unit, but my doctor, physical and occupational therapists, daughter, social worker and I agreed I needed more care. So I’ve lived here ever since. Visiting with a neighbor. I like everything there is to like about Kethley House. The staff is top notch. I am lucky Geneva [Forté, activities specialist] helps me interpret my needs. When she’s not here, I write my requests out to the other staff. My neighbors are very nice. I enjoy going to the activities with them. When I’m not doing more social things, I read, or watch T.V. in my room. I attend the services at the Chapel. I was raised Baptist, but sometimes attend the Catholic Mass. When I first moved here, my daughter visited me a lot. But, I am sad to say, she passed away in February 2003. I love when my grandson brings my great-grandbaby by to see me. It’s hard to see them very often because my grandson works two jobs. But he makes time to visit me. ” 5 The Margaret Blenkner Research Institute conducts studies that explore and evaluate new approaches to improving services for older persons and their families. Findings are disseminated nationally and internationally. In October, Linda Noelker, PhD, Senior Vice President of Planning and Organizational Development at Benjamin Rose, was honored as one of 10 Distinguished Women in Healthcare by the Visiting Nurse Association Healthcare Partners of Ohio. The annual awards recognize women who have made significant contributions in the health care industry. Dr. Noelker was recognized for her career contribution to health services research. Dr. Linda Noelker. * * * Administrators of CCRCs (Continuing Care Retirement Communities) need to promote 6 CCRC project meeting. positive resident-staff interaction, and integrate the services residents deem most valuable. These were the findings by Margaret Blenkner Research Institute staff members Dr. Farida K. Ejaz, Dorothy Schur, and Kathleen Fox in an AARP Andrus Foundation-funded study. The study examined satisfaction of 137 CCRC residents, most of whom lived in independent living units. Cleaning (93%) and having someone check on the residents (77%) were the services most often used. * * * The Chronic Care Networks for Alzheimer’s Disease Initiative is nearing completion. This six-year project developed and tested an innovative model of primary and long-term care for Alzheimer’s disease and other dementia. The Margaret Blenkner Research Institute spearheaded the national evaluation with support from The Robert Wood Johnson Foundation, Retirement Research Foundation, and Alzheimer’s Association. Findings show the care model led to a variety of positive outcomes for providers, patients, and family caregivers. Replications are being planned and will continue to improve services for older adults and families dealing with dementia. Claudia Ford, participant, Project PLAN Claudia’s Story: “ A Benjamin Rose staff member approached me about taking part in a caregiver research project conducted by the Margaret Blenkner Research Institute. Since my background includes program development and quality assurance, cultivating new ideas comes naturally, along with a passion for learning. I’ve been the primary caregiver for my mother for eight years. My parents live with me. My father and I are a team, helping my mother. She is extremely healthy, yet has early-stage dementia. I found this study, “Planning for Late-life Activities and Needs” (Project PLAN), to be extremely helpful. Because for me, the most challenging aspect of being a caregiver is the emotional strain of having other people evaluate how you’re carrying out your role. The social worker did an incredible job developing a rapport with us and supporting my goal: to give back to my mother the freedom and confidence that she gave me. My mother and I were given a wide latitude to discuss “differences” in our values. There was depth in the interviewing process. We were able to talk at great length about planning. Before the study, my mother had been confining herself to the house. But her involvement in the study increased her confidence, selfimportance, and independence during the exercises and one-onone talks. Through my eyes, caregiving is a very spiritual process. You can expect tough times if you’re not willing to reach for the deepest level of understanding … of yourself, your goals, and the person you love. Discussing care planning. Over 50 caregiver/care receiver pairs were interviewed in 2003 for Project PLAN and over 20 more will be interviewed in 2004. The three-year project is led by the Research Institute’s Dr. Carol Whitlatch in partnership with The Gerontology Center at Pennsylvania State University and funded by the Administration on Aging. Results of Project PLAN will be presented at national and international conferences and will help in the design and development of interventions for caregiving families. ” 7 Margaret Wagner Apartments opened in 2003 as supportive housing for the elderly. The 24 units represent the latest use of Benjamin Rose’s former nursing home, now a multi-use building. An evolving history: 1961 Named for Benjamin Rose’s first executive director (pictured above), Margaret Wagner House opens as a state-of-the art nursing facility. 1997 Residents move to Kethley House, as Margaret Wagner House can no longer provide the various types of care necessary. Concordia Care opens on the first floor as a day health center that enables frail elderly to remain in their own homes. Along with that a man from whom I had leased cars suggested I come to work selling cars. Other than putting gas in them, I knew nothing about cars. I scored so high on the aptitude test the manager couldn’t turn me down. I have worked in retail and as a volunteer since then. Barbara’s Story: “ I was born in Cleveland and grew up in Cleveland Heights and University Heights. I graduated from Cleveland Heights High, then The Ohio State University in 1955, getting married 12 days later. I worked in a bank before having children, after which I stayed home. My husband died suddenly when my second son was nine weeks old. I used to take walks with a neighbor and our children. We would talk about going into business together—someday. One day she called to say she had rented a store and asked what kind of business should we open? We began a consignment shop for antiques and “things too good to give away.” It grew rapidly and we moved to larger and larger stores, spanning 12 years. We never made much money but enjoyed our work, and the consignors were delighted. It was after we closed that a gentleman (who was a customer and friend) suggested that we begin an appraisal company (of Joking with the horticultural therapist. There are various activities available such as the book discussion meetings and the horticultural therapy program.
