OSITION

:

Under the oversight and support of the Community Mission Worker, this position seeks to facilitate the provision of intensive assistance to people who face multiple barriers to finding employment by assessing needs and concerns and providing a broad range of flexible assistance through individual case management and the development of an intervention plan. Participants will be linked with government services, a broad range of service providers and community organisations.

Ensure the understanding of, and compliance with The Salvation Army policies and

procedures (as amended from time to time) including those on Occupational Health,

Safety; Rehabilitation, EEO; Industrial Relations; Information Technology and

Financial Management.

Oversee the implementation and direction of the Participate in Prosperity (PiP)

Program at Lifehouse and provide timely program progress reports to management

Ensure an effective team approach to program implementation and service delivery.

Ensuring that the delivery of the service provided meets the requirements of the

Memorandum of Agreement.

Provide peer support/supervision to other case workers to ensure they are handling each case correctly.

Negotiate combinations of therapeutic interventions, educative and practical services

with service users and link them with services and resources within their community.

 Ensure individual intervention plans for service users, including those with more

complex support needs are developed, planned and implemented and evaluated within

organisational requirements and funding guidelines.

Attend case management meetings with clients, other professionals and stakeholders

as needed

Organise, Prepare and Facilitate group sessions, activities and workshops for participants

 Develop and enhance networks in the community to ensure that service users are effectively referred to appropriate support

Make appropriate placements of participants into employment, work experience, education, training and other relevant placements.

 Maintain confidential, relevant participant files in accordance with Salvation Army and DEEDI guidelines

Maintain up-to-date participant data on SAMIS

Provide reports and participant information to DEEDI in a timely manner

 Promote, develop, and maintain productive, amicable working relationships with diverse individuals and groups, including peers and supervisors.

 Ensure compliance with The Salvation Army guidelines regarding intake, case management and outreach practices.

Essential Requirements:

A commitment to the Christian faith and mission of The Salvation Army. An ability to offer pastoral assistance, to pray and to explain the Christian Gospel.

Demonstrated knowledge and understanding of issues confronting long term unemployed with multiple barriers

A demonstrated commitment to the Vision and Values of The Salvation Army Forest Lake.

Demonstrated ability to co-ordinate, implement and monitor effective case plans

Demonstrated ability to lead and work within a team

Well developed and effective communication skills (listening, verbal and written)

Demonstrated interpersonal negotiation and problem solving skills
