JOB DESCRIPTION PRO YOUTH CASE MANAGER OUTREACH PROGRAM GENERAL DESCRIPTION The Eastside Outreach Case Manager is responsible for providing direct services, including street outreach and case management as needed to runaway/homeless youth ages 15-22. Other responsibilities include client advocacy, developing community linkages and resource referrals for underserved and hard to serve youth. Community outreach to schools, businesses and other organizations interested in helping youth will also be required. The Eastside Outreach Case Manager will work with a team of two part-time Peer Leaders and one other full-time Case Manager. The Outreach team will be part of countywide outreach project and will meet with all county teams and Project Coordinator once a month. FLSA STATUS: Exempt REPORTS TO: Outreach Program Coordinator JOB RESPONSIBILITIES A. Case Management 1. Enroll homeless/runaway youth into case management as needed. 2. Assist clients in developing and meeting their individual goals. 3. Provide necessary referrals for services or agencies specifically designed to help clients meet their goals. 4. Advocate on the clients behalf when necessary to aid progress toward desired goals. 5. Maintain data collection and case records as required by the agency. 6. Utilize the expertise of enrolled youth in order to engage other homeless youth. B. Street Outreach 1. Locate runaway & homeless youth living on the street and/or living between friends, family members and willing adults. 2. Connecting with schools, teen centers, community businesses and other community services where youth are frequently located. 3. Providing necessary referrals and information for temporary housing and community resources. 4. Transporting youth to authorized safe, temporary shelter. 5. Interacting with youth in a culturally sensitive and respectful manner. 6. Supervise Peer Leaders. C. Community Outreach 1. Assist with presentations to school personnel, community businesses and other community services where youth are frequently located. 2. Maintain a professional demeanor. 3. Manage Teen Feed Program. JOB REQUIREMENTS A. Education and Experience 1. Bachelor's Degree in a social service related field. 2. Experience working with homeless youth and/or at-risk youth. 3. Education or experience in cultural competency. B. Knowledge, Skills and Abilities 1. Familiarity with Eastside resources. 2. Understanding of normal adolescent development. 3. Comfortable interacting with youth in informal settings. 5. Consistently maintain required paperwork. 6. Ability to establish and maintain trusting relationships (having patience & perseverance). 7. Ability to develop appropriate boundaries. 8. Must demonstrate sensitivity to needs of clients in crisis. 9. Employees and volunteers who operate their own or Friends of Youth vehicles on Friends of Youth business must have a safe driving record as defined by Friends of Youth. Employees and volunteers who operate their own vehicle(s) on Friends of Youth business must carry auto liability insurance that complies with the requirements of Friends of Youth. 10. Knowledge of relevant COA standards and participation in a Performance and Quality Improvement process. 11.
