MAIN PURPOSE OF JOB On behalf of NHS Kingston and the Royal Borough of Kingston: 1. Job Summary This post is at a strategic level to reduce inequalities in health experience and outcomes by communities living in areas of deprivation. Together with the Equalities and Community Engagement Manager and Public Health lead for inequalities, the post holder will identify specific priority groups residing in the borough and/ or areas in the borough where people are experiencing multiple inequalities in health and wellbeing. (Specific priority groups can and will include; the main areas of the borough where the Multiple Index of Deprivation has highlighted as most deprived. Residents, and community groups living in these areas the post holder will also link in with will include those with mental health problems, homeless people, Black and minority ethnic communities, disabled and older people, lesbian, gay, bisexual and transgender people, and other identified vulnerable or marginalised groups). It forms part of NHS Kingston and Royal Borough Kingston’s commitment to promote health and reduce inequalities, improve health services and capacity building within marginalised or vulnerable communities, supporting community groups and organisations to work with individuals to improve their health and access appropriate services. The post will work with statutory and non statutory service providers to develop appropriate access or referral systems with community groups, support early intervention and access to primary care services and social care. It will also assist the statutory and non-statutory organisations with delivering the equality agenda in health and social services locally. The post holder will be based in Community Care Services, (adult social services) and Public Health within a team of Community Development Workers also working towards eliminating inequalities in health experienced by marginalised groups. However, this post will be the only senior CDW post that will be specifically working with a remit across all areas of deprivation as well as supervising other CDW’s roles. The team is made up of 2 Senior CDWs, 3 other CDWs and the team manager. This post will have supervisory responsibilities for two of the Community Development Workers –the CDWs for two or three deprived areas of the borough. The post will be supported and steered by senior management staff working across Public Health, Adult Social Care, the Housing Department, the Mental Health Trust and voluntary sector service providers. The post holder will be provided with regular monthly supervision and will be required to work flexibly and independently on their own initiative to deal with complex issues facing ‘vulnerable’ groups and residents living in deprived areas of the community. The post holder will work in conjunction with other Community Development Workers within their own team as well as Community Development Workers based in the community. The post holder will be expected to work in line with and support Equality Schemes, Equality Groups and the Community Development Network. They will also support the implementation of, and adherence to, relevant health and social care policies. 2. MAIN DUTIES/ RESPONSIBILITIES To improve the provision and access to health, social care, housing and other community services available to residents by working directly with identified communities and relevant stakeholders. To co-ordinate community development work in the main 4 localities (areas of deprivation in the borough) and supervise staff responsible for delivering community development outcomes in each area. By working to community development support principles, the CDW also has four key components to the post and acts as a: 1. Change Agent e.g. by identifying gaps; developing innovative practice Assess needs of the local community, identify key areas for action and contribute to or design local action plans. Support the Team Manager with developing strategic and multi agency strategies and/ or policies to address needs. Establish effective community engagement activities and systems for services to engage communities and individuals Consult and involve community members to ensure their views, experiences and expertise are represented to service providers 2. Service Developer e.g. promoting joint working, education and training Develop and then co-ordinate a workable community development model for the four areas of deprivation in the borough Promote service provider knowledge and understanding of community needs to ensure appropriate services are available Facilitate information sharing and collaborative working with all relevant agencies and service providers but especially in primary care and health promotion. Support and develop health promotion initiatives within community based settings 3. Capacity builder e.g. Supporting voluntary groups and organisations to develop services Establish robust mechanisms to ensure the ‘public voice’ from all locality based CDWs is fed into local planning/ policy processes Influence commissioning decisions and/ or proposals by working with partners and communities Support the development of existing and new services provided with local voluntary sector organisations Support voluntary sector organisations and Faith Community groups to make appropriate referrals and attain support and advice from health, social care and housing providers when necessary. 4. Access Facilitator e.g. creating awareness of services and overcoming barriers. Support NHS Kingston colleagues to deliver health promotion programmes to relevant groups Identify barriers to accessing specific services and tackle these i.e. literacy barriers, low income and lack of understanding of service provision or what is available Assessing needs by carrying out or supervising needs assessment work Promote access by creating awareness of existing services both to community representatives, other community development workers, faith leaders and voluntary sector organisations. Co-ordinate work in deprived areas to do with a) health promotion initiatives and activities which address the wider determinants of health. Support service providers to carry out Equality Impact Assessments and ensure they provide culturally sensitive, appropriate services Other elements of this post Supervisory Responsibilities To supervise two Community Development Workers working towards reducing inequalities in two/ three locality areas. To provide 1:1 Supervision once a month and annual appraisal once per year to the two CDWs. With the Team Manager, plan the team plan and the CDWs work remits for each year to tie in with the team plan. Supervise and support the two CDWs to produce annual Aims and Objectives and review these for updating bi-monthly. To take part in monthly 1:1 supervision and annual appraisal with the line manager, produce aims and objectives for the year and update these bi-monthly. Information and Advice (i) To supervise the promotion of and provision of easily accessible, user-friendly, culturally appropriate information and advice services relating to all agreed aspects of health and social care services on behalf of statutory and voluntary health and social service providers. (ii) To establish links, and develop and maintain good working relationships with key people in heath and social care services, Housing and other Council departments and voluntary organisations who provide information to the public. (i) To initiate and lead on or supervise the co-ordinating and arranging of health promotion events to promote health and social care to the public/ potential users of services. (ii) To contribute to training programmes and attend staff meetings with the appropriate agencies to raise health and social care staff awareness about the various health and social care services, projects and sources of support available in the community and how they can be accessed. (iii) To positively promote and contribute to the improvement of health and social care information and services for people living in deprived areas and/ or from marginalised groups. General Duties (i) To liaise regularly with other providers of information within the Royal Borough of Kingston, including Customer Services, Housing and other Directorates and services provided and commissioned by Kingston Primary Care Trust and GP Practices. (ii) To draft reports as requested by the Head of Community Care Services, Equalities and Community Engagement Manager, the Lead Officer in the PCT, and relevant accountable groups and committees. (iii) To report and contribute to the running of the Community Development Partnership Group that reports to the Choosing Health Partnership Group (iv) To set up consultation forums and panels and arrange meetings between different agencies and/ or community groups. (v) To lead on relevant community projects as required for CCS and the PCT (vi) To implement and contribute positively to the development of the Council and Trust’s Equal Opportunities Policy. (iv) To act in accordance with RBK and Primary Care Trust Procedures. (vii) To assist in the regular reviews of the post and participate in its change and development as necessary. (viii) To work on own initiative and be responsible for carrying out tasks set without direct supervision.
