COMMUNITY HEALTH NURSE POSITION DESCRIPTION POSITION TO BE BASED AT: Warragul RESPONSIBLE TO: The Board of Management through the Director of Community Services. Additional qualifications in Community Health Nursing, Health promotion and/or Sexual reproductive health are desirable. Previous experience working with young people in rural is an advantage. Driver’s licence and use of own vehicle from time to time may be required. Specialist Scope of Practice: Pap Smear Provider, Asthma Educator/Respiratory Nurse JOB FUNCTION: To provide a clinic nursing support role at The Clinic, now located at headspace, which is an adolescent clinic for clients 12 – 25years. The role includes provision of information and referrals for young people of presenting issues, which include social issues and sexual and reproductive health issues. There is also opportunity to participate in the development and implementation of Health promotion activities, particularly in conjunction with the Health Promotion Team to schools and the community, as well as the provision of Asthma Education or Respiratory Nursing Services KEY DUTIES: Core Scope of Practice: 1. Provide a coordinating role to The Clinic, including, but not exclusive to ensuring good working relations with the GP’s, policy and procedure development. 2. To liaise with other health and welfare persons, GP’s, Medical Specialists, community and consumer groups, internally and externally, to ensure that services are developed to meet the needs of the client in a comprehensive and coordinated manner.(Case Management approach) 3. To provide counselling (nursing specific) and on going nursing support services to individuals, families and the community, as part of a multi-disciplinary service delivery approach, which may include Asthma or Respiratory Nursing Services or Sexual Reproductive Services. 4. Work towards a multi- disciplinary team approach, in working with clients, both individual and organisational. 5. To be responsible for the maintenance of your casework for those persons who are referred or present to the service. 6. To update casework files regularly (i.e. daily) ensuring all information pertaining to the client, either directly or indirectly; and all contact with the client, by phone or in person is recorded accurately and with sufficient detail to enable continuity of cases. 7. To ensure the data collection requirements are met, including accurate collection, entry to the data base, and analysis of work loads based on the data feedback. 8. To work to the established procedures for intake of caseload clients, development of case plans and discharge plans of clients, that is acceptable for accountability purposes. Specialist Scope of Practice: 9. To provide a Well Women’s Clinic which includes the provision of Pap Smears, Sexual Reproductive Health issues, and the provision of referral to appropriate women’s health issues providers, both on site and outreach to the Baw Baw Shire. 10. To provide Asthma Education Services/Respiratory Nursing Services HEALTH PROMOTION AND EDUCATION Core Scope of Practice 1. Assist in the development and delivery of appropriate evidenced based health promotion activities and health education programs for the service area which may be in conjunction with the Health Promotion Team. Taking into account the current trends and models of working including the DHS Service agreement Health Promotion Plan, Ottawa Health Charter, Social Model of Health, Health Promotion Framework the State and National Health Goals and Targets, and the local planning tools identifying community needs. In association with the Board of Management, to assist in the promotion of the concept and benefits of Primary Health Care, and the services it has to offer. Promote health, in its broadest sense using evidenced based information and targeting the needs of the community you are working with ie general community, schools, young people. Promote a collaborative multi-disciplinary approach to the development and implementation of Health Promotion activities. COMMUNITY DEVELOPMENT WORK: 1. To initiate and participate in community development and social planning projects in the service area which are appropriate and relevant to the goals, policy and target groups of the service. 2. Use Community Development Principles in determining the needs of the community. INFORMATION DISSEMINATION & NETWORKING: 1. Within the time constraints, to be responsible for encouraging greater community awareness of health issues, by speaking to groups, and collecting and disseminating information. 2. To maintain a health resources filing system and information sharing mechanism for Service staff. FORWARD PLANNING, RESEARCH & EVALUATION: 1. To assist to identify areas of unmet need in relation to health issues, within the service area and to assist in developing and meeting goals and targets set within the Health Service Agreements. 2. To assist the Board of Management, through research, with forward planning as required, particularly with regard to Health Service Agreements. 3. To adequately evaluate Health Education and Promotion Programs developed by the incumbent and document all activities ensuring the programs are evidenced based and well targeted. 4. To be prepared to undertake research as required. ORGANISATIONAL REQUIREMENTS: 1. To liaise closely with Service Staff and other staff working at the Centre to develop a multidisciplinary, approach to case work, to general health care, and health education, and promotion activities. 2. To participate in the training of students. 3. To maintain accurate records of daily activity in the format required for the current data collection system. 4. To provide regular reports to the Director of Community Services and other reports as required. 5. Attendance at staff meetings is a requirement of your employment. 6. To participate in the staff appraisal process “My Workplan” which will be based on your position description and your yearly targets and goals. 7. To participate in the Standards review Process, Accreditation program and other Q.A. programs undertaken by the Service. 8. To ensure you participate in relevant workshops and seminars to update skills in line with the Service Policy. 9. To work in accordance with current service goals and policies 10. To liaise with other Community Health Nurses and primary care services in order to promote professional development. 11. To perform such other duties as required/negotiated by the DOCS. SPECIFIC CONDITIONS: 1. That from time to time, use of private vehicles will be required, with prior approval from the DOCS for which reimbursement at the award rate will be paid. 2. The incumbent will be expected to participate in the professional supervision of their casework. 3. High level communication skills and ability to work autonomously within a multi disciplinary team. 7. Commitment to the principles of continuous quality improvement. 8.
