PERMISSION SLIP
Parents:
On May 11, 2010, students will be taking a field trip to Medieval Times in Schaumburg, Illinois.*
We will be gone a majority of the day and hope to return by 3:00. We will need only a few
chaperones for this trip. Unfortunately, this trip is our most expensive excursion this year:

Since the final cost above is more than many can afford, we are offering students an opportunity
to reduce this figure through the sale of candy bars. Candy bars will sell for $1.00. For every tote
sold, a student’s ticket will be reduced by $26.00. He or she may continue to sell candy bars until
his or her field trip is paid for in full. In addition, the cost of a chaperone ticket may also be
reduced in the same fashion. Ticket reduction will be only done in $26 increments. If students
sell an odd number of candy bars, the profit from those sales will be deposited in the seventh
grade team fund. Students will not get credit for partially sold boxes.
Please complete this form and return it by Thursday, April 15, 2010, so we can estimate the
number of candy bars we need to order.
If you are selling candy…The candy bar sale will be held from April 16 – April 26.. All money
and remaining candy bars must be returned by that date (April 26). During this time, money will
be collected daily before school by your child’s English teacher (Mrs. Mast). All money will need
to be paid by the morning of April 27th. We must send the check that day. Students who do not
return candy or money will not be allowed to attend this field trip and will be placed on a
debt/collection list.
If you are not selling candy…the money for the trip is due April 26th.
Since we only have room for a limited number of chaperones, please complete this form
immediately. Chaperone requests will be numbered in the order received and will have to be
limited to available seats. A chaperone ticket is $59.00.

*Behavior and classwork requirements must be met to participate in this field trip. Once
the money is sent to Medieval Times, no refunds will be possible. See the back of this
