Take dosage Examination #2 no later than March 2, 2009 Review examination #2 and review problems you had difficulty with (if any) with a dosage calculation specialist Review equivalents, abbreviations, and formulas to ensure accurate recall Continue to work sample problems suggested by dosage calculation specialists Take dosage examination #3 no later than March 26, 2009 If you PASS all three dosage examinations with a minimum of 80% or above on each exam (at least 36 correct answers out of 45 possible answers), you have completed the dosage calculation examination requirement for this course. Congratulations! You DO NOT have to take exams # 4 or #5. If you FAIL all three dosage examinations, you MUST take an incomplete in the course. This means that you can complete the theory portion of the course and complete the dosage exams next semester. If you pass two of the 1st three dosage examinations, consult with your instructor before you proceed. If you pass only one of the 1st three dosage examinations, consult with your instructor before you proceed. Attend the group workshop on April 4, 2009. If you have not taken an incomplete, and if you DO NOT pass 3 out of 5 dosage calculation examinations with 80% or above on each exam, you will receive a grade of F in the course even if you have a passing average in the theory portion of the course.
