NHS Greater Glasgow and Clyde is the largest National Health Service provider in Scotland with an annual operating budget of just under £2 billion. In recent years, NHSGGC has invested £15 million in new facilities and equipment, specifically in the areas of CT, MRI, Ultrasound, Digital Mammography, PET CT / SPECT CT, Angiography and General Digital Imaging. The South Sector includes the largest hospital in Scotland, the Queen Elizabeth University Hospital Glasgow following the closing of the Southern General Hospital, Victoria Infirmary and Western Infirmary and merging of services. Together with Gartnavel General Hospital and the Victoria ACH this will form the new South West Sector. Applicants must have full registration with the General Medical Council and be eligible for inclusion in the GMC Specialist Register. Those trained in the UK should have evidence of higher Specialist Training leading to CCT or be within six months of confirmed entry from the date of interview. The Hospital Modernisation Programme - The Services of Tomorrow A massive re-organisation of NHSGGC has recently been completed. As such, the Diagnostics Directorate took responsibility for all Diagnostic services in February 2006. The re-organisation was essential to align the organisational structure with the Acute Services Review (ASR) recommendations and support the Hospital Modernisation Programme. This is transforming healthcare provision locally, regionally and nationally. More than seven hundred million pounds of investment underpins an ambitious building programme, designed to deliver world class and integrated care from the following major acute sector units: · · · · · New Cancer Centre, PET CT Imaging Centre, on the General hospital campus at Gartnavel, opened in 2007. Development of a single dedicated Regional Cardiothoracic Centre at Golden Jubilee Hospital, completed 2008. Ambulatory Care, Diagnostic and Treatment Centres at the Stobhill and Victoria sites, opened June 2009. New Acute South Glasgow Hospital with co-location of Maternity, Children’s and Adult Hospital services. Regional Neurosciences and Maxillofacial Centres are also on site. Due for completion in 2015. Re-development of Glasgow Royal Infirmary into the second major acute hospital from 2015. The Hospital Modernisation Programme will ensure that walk-in/walk-out hospital services are provided for the majority of patients. The pattern of service provision will shift to reflect moves towards ambulatory care. Currently 85% to 90% of patient encounters with acute hospital services are on a same day basis. These include outpatient attendances, diagnostic tests, imaging procedures, and a range of day surgery procedures. In future, these services will be provided from ambulatory care centres designed to deliver a streamlined and rapid process of care. The redesign and redevelopment of Glasgow's acute services will address many of the pressures currently facing the hospital service. The new services will be provided in modern facilities rather than in early 20th century buildings. The purpose-designed facilities will enable the one-stop/rapid diagnosis and treatment models required for the future. State of the art IT services and PACS currently allow patients’ images and diagnostic results to be available throughout the city and beyond, regardless of which hospital site is providing services. Concentration of inpatient facilities into fewer sites across the city will satisfy the requirements of junior doctor’s hours and issues arising from increasing sub-specialisation of medicine to be addressed, through the creation of larger staff teams and sustainable rotas for both junior and senior staff. 3. The key strategic aim of the Directorate is improve the quality of care provided to patients on the basis of clinical need by consolidating acute/emergency imaging support on a 24/7, 365 day basis and by complying with emergency, cancer related and scheduled care National waiting times targets. The NHSGG out of hours imaging model for the adult acute service includes robust vascular/interventional on-call cover city wide with plans at an advanced stage for a centralised diagnostic on call service centred at the Royal Infirmary covering all Glasgow and Clyde acute sites. Consultant Radiologist “out of hours” activity will be incorporated into job plans as a mix of evening/overnight “on-call” cover, weekend daytime activity and weekend daytime “on-call” cover Thus within a framework of recognised contractual hours, there is likely to be a future change in the current working practice of routine 9 to 5pm, five days a week working, to one in which extended working days, and weekend working will become the norm. 6. Education & Training Close links to the University of Glasgow ensure significant engagement in undergraduate and postgraduate clinical teaching. The NHSGGC Imaging Directorate is the major clinical sponsor for the West of Scotland Radiology and Nuclear Medicine Postgraduate Training Scheme and around 40 Specialist Registrars currently rotate through the Glasgow departments. The Glasgow Caledonian Course in Diagnostic Ultrasound is accredited to Masters Level and trainee Sonographers are educated and mentored locally. The NHSGGC Imaging Directorate also hosts the postgraduate training programme for clinical scientists, technologists and radiographers. 7. Glasgow is the lead site for the Scottish National PACS implementation and completed National acceptance testing in January 2007, triggering the Glasgow wide roll out which was completed in mid 2008. This has undergone a recent upgrade to V11 with replacement of hardware and refreshing of the software package. Voice recognition technology has also been introduced at all Glasgow and Clyde hospital sites along with upgrades to the RIS/PACS systems. The Trak-Care PAS was introduced in 2013 and electronic requesting with “Order” Comms has been in operation across all sites since May 2014. All acute adult and paediatric hospitals and the ambulatory care hospitals have provision of modern mulltislice CT and 1.5T MRI systems. In addition, a 3T magnet is available at the regional neurosciences unit on the Southern General campus. A Regional PET CT Centre, with two modern PET-CT scanners, has been developed adjacent to the New Cancer Centre. Dedicated vascular/interventional facilities are available at Queen Elizabeth University Hospital (QEUH), Gartnavel, Glasgow Royal Infirmary, and in Neuroradiology/INS. Vascular/Interventional facilities in The Royal Hospital for Children are shared with Cardiology. Total imaging examination numbers for Glasgow approach over one million per annum (excluding Obstetric US and Cardiology). The annual incidence of cancer for the Glasgow area is in the order of 7,000 cases. Regional services cover up to half the population of Scotland, potentially doubling the caseload for some patient pathways. The Diagnostics Directorate currently employs around 116 individual Consultant Radiologists across Glasgow: North East Sector South/NW Sector Neuroradiology Paediatric Radiology Breast Screening Dental Hospital Clyde 23 (including vacancies) 46 (including vacancies) 10 7 7 2 21(including vacancies) Around 40 Specialist Registrars rotate through the West of Scotland Training Scheme. South Sector The construction of the QEUH, linked to the Institute of Neurological Sciences, the New Children’s Hospital and the redeveloped Maternity Hospital is at the core of the NHSGGC Modernisation Programme. The transfer of acute services from the existing sites: SGH, VIC, WIG, GGH was completed in Spring 2015. The QEUH hospital campus is the largest acute hospital site in Scotland providing inpatient services for the population of South Glasgow and well as some city wide services including vascular, renal and regional services. There are 5 CT scanners and 3 MR scanners between adult and paediatrics as well as a modern nuclear medicine department and this excludes additional provision at the Neurological Institute. The main clinical campus for Glasgow University and the laboratory services for Glasgow are also relocating to the South Glasgow campus and substantial investment in research and education facilities is underway. The Victoria Hospital ambulatory care facility, opened in June 2009 and provides modern facilities for diagnostic procedures, day surgery and ‘walking wounded’ casualty services. Facilities within the Victoria ACH include a 64 slice CT Toshiba scanner, 3 ultrasound rooms with dedicated breast ultrasound and elastography and liver imaging software, a Siemens digital stereotactic and digital breast mammography unit and a multipurpose fluoroscopy unit. Two 1.5T Philips MR scanners with whole body imaging, a full range of phased array multichannel surface coils and a breast coil with biopsy capability is available. There is full RIS/PACS integration. Further floor space allows for expansion of CT and other services/equipment. Departmental library and seminar rooms are available for MDTs and teaching, in addition to shared Consultant office facilities. Adult and Paediatric Nuclear Medicine Departments in South Sector have 4 gamma cameras (3 SPECT-CT in Nuclear Medicine, and 1 dedicated brain SPECT at Institute of Neurological Sciences. North West Sector The Western Infirmary and Gartnavel General Hospital jointly constitute one of the teaching hospital units linked to the University of Glasgow, the main campus of which is currently adjacent to the Western Infirmary site. As part of the Glasgow Modernisation Programme, some services have already centralised to the Gartnavel site. These include the Infectious Diseases Unit, the Ophthalmology service and ENT. Specialist inpatient breast services are expected to be sited at Gartnavel in the long term. The Western Infirmary is due for closure with out patient facilities being available in GGH and the previous Royal Hospital for Sick Children to be called the West Ambulatory Care Hospital. The new Beatson West of Scotland Cancer Centre opened in 2008 at the Gartnavel campus. Details can be found on the website www.beatson.org.uk. West of Scotland PET Centre at Gartnavel General Hospital is equipped with two PET-CT scanners (GE Discovery 690 and 710) and is one of the busiest clinical PET CT services in the UK. It is served by the onsite cyclotron unit. West of Scotland PET R&D group is closely linked with the Glasgow University preclinical PET facilities. Gartnavel General Hospital Nuclear Medicine Department provides diagnostic services as well as benign and malignant radionuclide therapies. It has three gamma cameras (one SPECT CT and two SPECT). Both PET CT and Nuclear Medicine departments have strong research activities mainly in oncology but also in non oncology trials including amyloid brain PET. North East Sector Glasgow Royal Infirmary is a large teaching hospital serving a local population of approximately 400,000, and providing supra-regional and national tertiary referral services, including Plastic Surgery, Upper Gastrointestinal Surgery, the Hepatobiliary and Pancreatic service and the Centre for Rheumatic Disease. It is one of two major acute sites being developed within the city. (The other is at the QEUH). Outpatient Imaging services are co-ordinated between the Royal Infirmary and the satellite Ambulatory Care Hospital (ACH) three miles North at Stobhill. This opened in May 2009 and provides modern facilities for diagnostic procedures, day surgery, renal dialysis and ‘walking wounded’ minor injuries services. Nuclear Medicine services in North East Sector are provided at Glasgow Royal Infirmary and Stobhill Hospital. Glasgow Royal Infirmary Nuclear Medicine has two SPECT CT scanners and one DEXA scanner, and provides diagnostic services as well as radioiodine thyroid therapy. It is the main centre for West of Scotland Nuclear Medicine Specialist Training Programme. Nuclear cardiology service is provided in a separate department at Glasgow Royal Infirmary and has three scanners for dynamic and pharmacological stress imaging as well as cardiac function. Stobhill Hospital Nuclear Medicine has one SPECT CT scanner and provide outpatient diagnostic scanning service. Clyde Sector Consultant Radiology provision within this Sector has been organised and modernised to support the future imaging requirements for the population of this region which is 400,000. A single area wide amalgamated and enlarged radiology team provides radiology services to each of the Sector hospital sites namely: the Royal Alexandra Hospital, Paisley, Inverclyde Royal Hospital, Greenock and the Vale of Leven District General Hospital, Alexandria. 10. The Job Itself Consultant Radiologist with an interest in PET CT/NUCLEAR MEDICINE Imaging This will be based within the South Sector – QEUH and GGH The job is advertised as a full time position although candidates who require to work part-time will be considered. The basic contract will be for a 10 PA consultant contract. EPA(s) will be the subject of detailed job planning discussions. Other current full time post holders have either 11.5 or 12 pa. The candidate will be expected to contribute strongly to the general work of the department, in particular the acute service. He/she will be proficient in a variety of diagnostic imaging including PET CT and general nuclear medicine. A sub-specialist interest in PET CT/NUCLEAR MEDICINE imaging is required to complement the existing contributing radiologists. There would be scope to support candidates wishing to develop supplementary interests in oncology imaging. Interventional (non-vascular) skills, including image guided drainage and biopsy are a basic requirement for the general radiology commitments. In line with the acute services review inpatient services are being rationalised across the city and as this occurs the job plan will adapt to ensure the applicant is working effectively and efficiently with colleagues to ensure high quality services across Greater Glasgow and Clyde. Further aspects of the job are negotiable and to some extent will be tailored to the abilities and interests of the successful applicant. However, the duties will include the supervision and reporting of CT and MR examinations, ultrasound, fluoroscopy, plain film reporting and general radiology on call duties. There is an essential requirement to be proficient in emergency and acute trauma radiology for emergency daytime and on call purposes. The successful candidate will participate in the diagnostic out of hours system on a sector basis. The maximum number of weekends expected to be contractually covered by any individual consultant will be no more than 8 per annum. Reconfiguration of Acute service in Greater Glasgow & Clyde may in future result in changes to on-call payment and commitment. All out of hours activity will be undertaken on the basis of the terms and conditions set out in the consultant contract (Scotland). The sector operates a compliant Specialist Registrar on-call rota, as laid out under the European Working Time Directive. The exact timetable will be subject to negotiation and current service needs. Clinical Commitments The weekly Programmed Activities (PA) will include: PET CT Nuclear Medicine CT/Duty MRI Flex Plain Film Oncall SPA 3 PA 1.5 PA 1 PA 1 PA 0.5 PA 0.5PA 0.5 PA 1 PA (+1 negotiable see below) *0.5 DCC per week is awarded for cross-cover of colleagues for leave. This job plan is negotiable and will be agreed between the successful applicant and the Clinical Director. NHS Greater Glasgow & Clyde initially allocates all full time consultants 10 PAs made up of 9 PAs in Direct Clinical Care (DCC) and one core Supporting Professional Activities (SPA) for CPD, audit, clinical governance, appraisal, revalidation, job planning, internal routine communication and management meetings. The precise allocation of SPA time and associate objectives will be agreed with the successful applicant and will be reviewed at annual job planning. Additional EPA activity This additional activity is separate from the main job plan contract and variable at 3 month notice period from either employee and employer. The work performed within these sessions will be general/cross-sectional imaging and composition may be varied with a reasonable notice period to permit flexible working to meet the service needs of NHS GG & C. On-Call On call is shared between consultant colleagues supported by radiology registrars. The directorate is keen to develop extended working in radiology to accommodate the increasing workload and a pilot is currently underway. This involves 3 consultants and 2 registrars working during the day at weekends and 1 consultant each night from 5-8pm. This predominantly involves provision of a busy in patient CT and ultrasound service and ensures rapid turnaround of the acute plain film service. In addition, a city-wide on call reporting hub staffed by radiology registrars 5pm-9am, receiving and reporting calls out of hours from all NHS GG&C hospitals commenced in February 2015. There is a separate Interventional Rota, but there is an expectation that general CT and ultrasound drainage will be covered by the diagnostic consultants. Teaching, Audit and Research There are traditional close links between NHSGG&C and the University of Glasgow Medical School comprising the Medical, Dental and Nursing Schools and the Graduate School. Participation in undergraduate, postgraduate teaching and clinical audit may be agreed with the appointee permitting more SPA time for specific clearly identified additional supporting activities. These activities have to be agreeable to the candidate and desired by the Board.
