Description
The resident will be exposed to three types of practice: a typical consultation
community office-based practice; a specialty practice with adolescents and adults
with ADHD in a clinic format; and a research-based practice that involves
medication trials and cognitive protocols. The student will have a solid
understanding of the field. In addition they will be encouraged to attend sessions
in Thunder Bay (for free), community in-services, school meetings, day treatment
program children, and didactic lectures. The research team includes a weekly
conference call with every major research-based ADHD team in Canada by
conference call. This selective is best suited for someone thinking of an academic
career in child psychiatry.
Residents may select a consultation experience in forensic child psychiatry. Dr.
Lindley Bassarath and his multidisciplinary team provide clinical consultation to
courts, judges, probation officers, agencies, and parents of antisocial youth.
Typical presenting behaviours include theft, assault, breach, truancy, arson, and
sexual offences. Typical diagnoses may include Conduct Disorder, Oppositional
Defiant Disorder, Substance Abuse, Executive Functioning deficits,
depression/dysthymia, Learning Disabilities, or ADHD. Opportunities for
telepsychiatry consultation are also available. Integration of a mental health
and risk assessment protocol is central to the assessment process. Ongoing groups
for parenting adolescents may also be offered.

This is a clinical elective. It can be tailored to fit the trainees’ particular needs. It
consists of either inpatient or outpatient experience dealing with adolescents who
have psychotic disorders, mood disorders or anxiety disorders including OCD.
Pharmacology consultation training is also available as in special circumstances in
a school consultation experience.

This elective is an out-patient selective that provides an opportunity for a resident
to participate in the assessment of disturbed adolescent outpatients in the context
of a multidisciplinary clinical research unit which specializes in the investigation of
affective disorders. He/she will be trained to use structured interviews for Axis I
disorders and an assessment of Ego Functions and learn how to integrate the data
which are collected with both clinical formulations and research hypotheses. The
resident could join an ongoing project which is looking at a number of parameters
of functioning in depressed adolescents, or simply use the experience to enhance
his/her diagnosis skills in his age group. There is also the potential to pick up
suitable cases for treatment, if desired.
This elective can be individually tailored to residents’ interests and can involve
exposure to different clinicians. The experience can include one or more of the
following: (1) participation in the clinical assessment with children, adolescents
and families; (2) participation in community consultation; (3) participation in a day
treatment program; and (4) individual and/or family therapy with supervision.

This elective will provide the resident with an opportunity to explore child
psychiatric problems for which neuropsychological and psychopharmacological
techniques have been found appropriate. The resident will participate in clinical
assessments of children with problems such as Attention Deficit Hyperactivity
Disorder, Tourette Syndrome, childhood Anxiety Disorders and a variety of
cognitive, learning and behaviour problems in children and youth. Depending on
the resident's interests and skills, the resident may learn to titrate and manage
psychostimulant treatment for children, and provide parent counselling to assist
parents in child management and advocacy in the school setting for their child
with learning difficulties. A resident may also participate in the concurrent
research of the Neuropsychiatry Team at the Hospital for Sick Children which
includes diagnostic, genetic, neuroimaging, and follow up studies of children with
ADHD and associated disorders. This elective may span the entire academic year,
and may count as two elective “blocks”. Commencing in 2008 the Royal College
will expect advanced competency in the management of ADHD for certification as
a Psychiatry Generalist. This elective is positioned to help residents achieve this
training objective.

This elective experience offers opportunity in mental health work within the
educational system. The setting in schools is universal and targeted school/mental
health programming to reach aggressive children in the school system. The
program operates influencing, involving and resourcing teachers, school
administrators and all students regarding the etiology of aggressive behaviour and
how to respond to increase pro-social behaviour. Clinical aspects of the selective
involve development of expertise in designing and implementing whole class
strategies which create a nurturing and psychologically reflective school
environment in which the aggressive child and all students learn academically and
socially.

This elective offers residents the opportunity to participate in a Day Treatment
Programme for adolescents with drug and alcohol problems. This resident will
have the opportunity to participate in the assessment of the youth’s needs as well
as in the development of practical and useful psycho-educational interventions for
all participants in the programme using the "Harm-Reduction" model.

The Gender Identity Service is lead by Dr. Ken Zucker with psychiatric consultation
from Dr. Susan Bradley. Presenting problems include: wish to be the opposite sex;
cross-dressing; cross-gender emulation of roles in fantasy play; anatomic
dysphoria; and paraphilic sexual arousal. Specialized assessments of children and
adolescents with gender identity disorder are conducted; in addition, as
adolescents with transvestic fetishism with or without co-occurring gender
dysphoria are seen. Youngsters with psychosexual problems secondary to a
physical intersex condition or youngsters with sexualized behaviour problems are
also assessed. Numerous case consultations to professionals in Canada and the
United States are done.
Clinical assessment is organized by a conceptual framework, which relates to
phenomenology, natural history, associated familial factors, and biomedical risk
factors. Standardized assessment tools are used as part of the clinical assessment,
including psychological testing with the child or youth and parental assessment.
Treatment is a combination of individual psychotherapy, parental counselling,
interventions in the naturalistic environment and, when appropriate,
pharmacotherapy for comorbid disorders. Residents may also participate in the
multidisciplinary intersex clinic at HSC.

An elective in interviewing skills with children, adolescents and families. This
elective will focus on a wide range of clinical cases, each presenting diverse
interviewing and assessment challenges. At the conclusion of the elective the
resident will have acquired skills in focused interviewing across a range of
psychiatric disorders. The resident will have acquired the essential skill of
understanding the critical issues driving the clinical presentation and positioned to
formulate appropriate intervention.

We can offer a range of experiences to residents, depending on their interests and
experience. We see children up to the age of five years with diagnoses including
Parent-Child Relationship Disorders, Feeding Disorders, Attachment Disorders,
Sleep Disorders, Pervasive Developmental Disorders, and Post-Traumatic Stress
Disorders. We offer a range of treatments including: behavioural psychodynamic,
family and mother-infant relationship therapies. Electives can be designed to suit
the individual resident.
This elective provides experience in consultation to an intensive behavioural
treatment class which is unique in its collaborative work between C.M. Hincks and
the Toronto District Board of Education. Residents will participate in assessments
of children with a range of disruptive behaviour disorders, developmental and
learning problems.

The Mood and Anxiety Service will be available for senior selective residents and
we do have more than one supervisor available. The clinical population is children
and adolescents (chiefly 6 thru 17 years) referred with possible mood or anxiety
disorders. Presenting problems include: anxious/depressive symptoms, suicidal
behaviour, manic and psychotic symptoms, unexplained somatic symptoms,
school refusal and declining academic performance, social isolation and peer
relationship problems, family relationship problems. Typical diagnoses include:
Generalized anxiety disorder, Social phobia, Specific phobia, Post-traumatic stress
disorder, Panic disorder, Agoraphobia, Obsessive-compulsive disorder, Major
depressive disorder, Dysthymic disorder, Bipolar disorder 1 and 2, and
Cyclothymic disorder.
The service provides direct care, shared care with referral sources and a
consultation service. The resident would have opportunity to be trained in CBT
and will gain experience in using a variety of medications. Although there is
genetic research going on in the service, clinical research is the most readily
available research experience. In particular, the interested resident could learn
about clinical trial methods. He/she could also receive supervision to support
development of teaching skills.

This elective offers residents a clinical experience providing psychiatric
consultation and follow-up care to homeless adolescents. Covenant House is a
youth shelter located in the inner city. It provides a variety of services to
homeless youth, including basic shelter, social work support, transitional housing,
a section 20 school, and health care. Residents will have the opportunity to assess
and treat street-involved youth with a wide range of diagnoses such as first onset
psychosis, conduct disorder, and substance abuse.

This elective offers experience in community based treatment through designing
in-classroom interventions for schools to aid emotionally disturbed children who
are in regular classrooms. School climate, peer pairing, ongoing teacher education
and whole class intervention are the primary modalities for change. Experience
can be gained with ADD, Conduct, ODD and Anxiety Disorders. There is
opportunity for the resident to gain skill in consultation, and multi-model
treatment design with a research evaluation component.

This elective offers residents the opportunity to explore in depth the psychiatric
issues and the complex dynamics of the "conduct disordered youth" (12-16 year
old males and females) while at the same time having the opportunity to examine
"institutional life" and its impact on developing effective treatment strategies
through various modalities. The Syl Apps Youth Centre contains a detention
programme, a closed custody programme and a secure treatment programme.
