SUMMARY OF UIC DOCUMENTS
1. Continuing Medical Education Activity Proposal – Episodic Activity
Description: This is the initial and renewal application for short duration single CME events. It contains the
following links that will be embedded in the online document:
a. Joint Sponsor Letter of Agreement for jointly sponsored activities
b. List of Verbs for Writing Objectives to aid activity directors in the writing of measurable objectives.
c. Disclosure of Interest form for speakers and all others who control content of activity.
d. Resolution of Conflict form to be completed by activity director when review and possible revision of
content is required.
e. Commercial Support Letter of Agreement to be completed when there is any commercial support of the
activity.
f. UIC CME Brochure Guidelines to be followed by the activity director when designing promotional
materials.
g. Session Evaluation must be completed by learners immediately following the activity to collect
essential data related to analyzing the impact of the activity and screening for commercial bias
h. Outcomes Samples Template to aid activity directors in designing instruments for tracking outcomes of
their activity.
2. Continuing Medical Education – Regularly Scheduled Series Curricular Planning Proposal
Description: This is the initial and renewal application for recurring activities including grand rounds, case
conference, tumor board and M&M. It contains the following links that will be embedded in the online
document:
a. Joint Sponsor Letter of Agreement for jointly sponsored activities
b. Regularly Scheduled Series Individual Event Planning Document to complete for each session of a
regularly scheduled series.
b. List of Verbs for Writing Objectives to aid activity directors in the writing of measurable objectives.
c. Disclosure of Interest form for speakers and all others who control content of activity.
d . Resolution of Conflict form to be completed by activity director when review and possible revision of
content is required.

3. Continuing Medical Education – Regularly Scheduled Series Individual Event Planning
Document
Description: This form should be submitted prior to each event in an RSS.
It contains the following links that will be embedded in the online document:
a. Disclosure of Interest form for speakers and all others who control content of activity.
b. Resolution of Conflict form to be completed by activity director when review and possible revision of
content is required.
c. List of Verbs for Writing Objectives to aid activity directors in the writing of measurable objectives.
d. Commercial Support Letter of Agreement to be completed when there is any commercial support of the
activity.

materials.

4. CME Post Activity Report
Description: This reporting form is intended to be completed 2-4 weeks after an episodic event or
annually during an RSS. It is intended to fulfill the following objectives:
i. Provide data on the compliance of activities with criterion 4 that content matches learners scope of
practice.
ii. Provide data on compliance with standards for commercial support, specifically with regard to
completion of all disclosure forms, identification of financial relationships, reporting of relationships,
and perception by learners of bias.
iii. Provide narrative synthesis documenting how activity supports UIC’s CME mission (criterion 12).
It contains the following links that will be embedded in the online document:
a. Post Activity Report Table (Excel) to be completed for both episodic and RSS activities
b. Attendance List to be submitted so that attendees may receive CME credit.
c. Disclosure of Interest form for speakers and all others who control content of activity.

d . Resolution of Conflict form to be completed by activity director when review and possible revision of
content is required.
5. Session Evaluation
Description: This session evaluation, intended for both episodic and RSS activities, is designed to elicit
essential information to assess the success and appropriateness of the learning intervention. The
form is mandatory for CME credit to assure that the physician receiving credit either falls within the
target audience or reports how the activity will change his/her practice. Subjective ratings of the
event may be elicited using a separate instrument. The survey solicits the following:
a. The physician’s name.
b. The physician’s specialty. A list of all specialties targeted by the intervention will be included on the
form.
c. A request for the physician to note whether each of the learning objectives was achieved.
d. A request for the physician to document whether the intervention will influence practice and, if so, how.
e. A request to indicate detection of commercial bias
f. A request to identify gaps that could be addressed in future CME activities.
5. Sample Questions for CME Outcomes Instruments
Description: This document provides a sample of questions that activity directors may use for constructing
outcomes surveys to be administered when indicated at some time point, typically 1-4 months,
following the education intervention.
