Welcome Our Services - Core How to get your test results This leaflet is for both existing patients and those considering registering with us. It tells you about how to access our services and some general information about how our practice operates. If you live in our practice area and would like to register with us, please complete one of our registration forms that are available from the reception desk. Please provide proof of address and photograph ID. We serve the Islington area from Holloway Rd/Seven Sisters Rd to Stroud Green Rd/Hornsey Lane/ Isledon Rd. Our team includes three GPs, a practice nurse and a health care assistant, along with other visiting clinical staff. Please note that we are not a training practice The core services of general practice include the general management of acute and chronic medical conditions, health promotion, appropriate referral and urgent care. You can call the surgery for the results of any tests everyday between 10:00 AM – 6:00 PM on the general enquiries line – 020 7527 1502. Our trained reception staff will be happy to advise you. Practice Opening Times & Appointments Availability The practice is open every weekday Monday to Friday from 8.00am to 06:30 pm. Please ring for appointment When we are closed your telephone call will automatically be diverted to our out-of hours GP service HARMONI. Premises Hanley is a purpose build GP surgery and fully accessible to all disabled patients You can book an appointment either by telephone or in person. To book by phone, call the appointments line on 020 7527 1500. Appointments with a GP last for 10 minutes. In some cases a double appointment can be booked, for example for 6-week postnatal checks or if you need an interpreter. Please discuss with the receptionist if you think you may need a double appointment. If you are unable to attend an appointment, please give us as much notice as possible so that your appointment can be given to someone else. You can cancel your appointment by calling the cancellation line on 020 7527 1504 or using our text messaging service. If we have your mobile number you will get a text reminder of your appointment. It is your responsibility to inform the surgery if you don not wish to keep your appointment. Telephone Access If you would like to speak to a clinician for advice this is what we will do: Take your contact details, record your message and the clinician will return your call at the end of surgery. Please use the general enquiries line - 020 7527 1502. Home visits Visits are for those who are housebound or too ill to come to the surgery. If you require a home visit please make this request before 10:30 am, if possible, It is always best to see patients on the surgery premises where the facilities for examination and assessment are better, please attend the surgery if you possibly can. The practice premise is accessible for all disabled patients. Repeat prescriptions All requests for repeat prescriptions should be made in writing and either posted or delivered to the practice. No requests will be taken over the telephone (except in emergencies). You must allow 48 hours for your request to be processed. When making your request for repeat medication, please use the tear-off slip from your last prescription. If you have lost this, you should come to the practice and write your request on the pre-printed form provided. If you have any difficulties, reception staff will be on hand to assist you. Housebound patients can post their repeat request to the practice and should enclose a stamped addressed envelope. Minor Ailment Scheme (MAS) – minor ailments are conditions that can be self-treated with medicine available over the counter from your pharmacy. MAS allows you easier access to the same advice, treatment and medicines from your pharmacist as you would get from your GP, without having to wait for a GP appointment. Your local pharmacist Your local pharmacist will be able to give you free health advice at any time - you do not need an appointment. Many pharmacies operate extended hours on a rota basis. Ph: 020 7272 1850 Family Health Services What to do when the surgery is closed Other contacts The practice uses an out of hour’s doctor service to cover times when the surgery is closed. This includes all day Saturday, Sunday and Bank Holidays. If you call the surgery when it is closed, your call will be automatically diverted to our out-of hour’s service. If you cannot wait until the surgery re-opens and wish to speak to a doctor urgently, staff on the out-of-hours helpline will assist you. Alternatively you can call our out-of-hours service (Harmoni) direct on 020 7388 5800, if you wish. I want 24 hours a-day health information You can call NHS direct on 0845 46 47. The service is open 24 hours a day, seven days a week and is staffed by nurses. You can also call to get information about emergency dentists. For further information please visit: http:// www.nhs.uk I need emergency treatment Call 111 for medical help or advice. NHS 111 is available 24 hours a day, seven days a week. If you have a severe injury or a serious accident, you should call 999. The Accident and Emergency Department at the Whittington Hospital, Highgate Hill is closest to the surgery.* Details of WALK-IN-CENTRES can be provided on request. Patient Reference Group (PRG) We have an active PRG & would welcome expression of interest from any of our patients. Please contact Lisa Couzins who is the PRG lead for this group Medical Records The practice keeps computer and paper records in your medical files and is registered under and governed by the Data Protection Act. You have the right of access to view your records and to take copies, though a small fee is charged. Personal data is only seen by professionals at the practice, involved in your care. Anonymised data may be sent to Whittington Health for quality and monitoring purposes. Your medical records are likely to be part of national database. If you do not wish this to happen, please ask the reception staff or GP. If you require further information regarding primary medical services in the area, please contact Whittington Health FHS Contract Manager on 020 7527 1206 Patients who do not speak English: The practice books face-toface interpreters for patients that have difficulty communicating in English. Please ensure that you request this service when you book your appointment. We also have a number of staff who speaks the following languages: Urdu, Punjabi, Bangali and Swahili.
