PRE-ADOPTION INFORMATION
Before you complete this questionnaire, please note the following important facts:
 Rabbits are NOT low-maintenance pets. They require special food, appropriate housing, and lots of love.
 Rabbits are NOT appropriate pets for children, as their anatomy is subject to injury with improper handling.
 Rabbits do NOT like to be picked up and carried. Doing so improperly can cause injury to both parties.
 Rabbits can live for 10 years or more. They are a long-term commitment.
 Rabbits require specialized veterinary care by a qualified exotics veterinarian skilled in their care.
 Rabbits are natural diggers and chewers. Your house will need rabbit-proofing.
 Rabbits are intelligent and active. They must have AT LEAST 4-6 hours of unconfined play every day.

ADOPTION FEES
All our rescued rabbits are spayed (female) or neutered (male) before adoption, and many have received
extensive medical care to remedy the problems caused by their pre-rescue situation. Veterinarians in our area
typically charge $200 or more to spay a female, and $150 or more to neuter a male. Additional treatments mean
additional cost. Our adoption fees help us defray rescue expenses, and ensure they are going to a home that can
handle the potential expense of veterinary care and proper feeding and housing of a rabbit.
The H.A.R.E. adoption fee is $75 for a single rabbit, and $125 for a bonded pair.
If you have adopted from us before, there is a discounted adoption fee.

Thank you for taking the time to answer the following questions completely and honestly. By knowing a little
bit about you and the rabbit’s future home, we can help ensure that everyone is prepared for this happy event,
and will have many years of health, safety and loving companionship. We have set up the form to enter your
responses in BLUE for ease of reading your answers. So please answer each question in the space provided
below the question.

13. Please describe where will the rabbit live (size of hutch, running area, etc.):
14. If you plan to keep the rabbit in a hutch, how much free running time will you provide per day?
15. Your house may need to undergo some special "bunny-proofing" (such as protecting electrical wires from gnawing teeth) before
the rabbits are allowed to run free indoors. Are you willing to take time to do such "bunny proofing?"
16. Would you be willing to allow one of our volunteers help you design a “bunny safe” environment in your home?
17. Some rabbits, like any companion animal, can be destructive when bored or not given enough attention. Would destructive
behavior (such as furniture chewing, carpet digging, less-than-perfect litterbox habits etc) be a reason for you to give up the rabbit
in the future? Please feel free to explain your answer.
18. Our adoption fee helps us defray the costs of spay/neuter, medical treatment, food and care of our rescued rabbits. Do you feel
that you can comfortably manage the expenses of
 adoption fee ($75 or more, depending on the number of rabbits adopted)
 toys (There are plenty of free toys you can make, but bought toys may cost a total of $50 or more)
 fresh timothy or other grass hay (~ $20/month per rabbit)
 daily fresh greens (~ $5-10 per rabbit week)
 enclosure hutches, pens, or pet gates ($30) (optional, but may cost anywhere from $30 - $500)
 veterinary care (variable, but probably no less than $150/year per rabbit)
19. Veterinary care is a necessary part of living with any companion animal. Although they do not require vaccinations, rabbits need
annual veterinary checkups performed by a qualified veterinarian experienced in rabbit medicine.
Serious medical conditions can cost hundreds of dollars, just as they can for a dog or cat. A well maintained rabbit may live 1015 years or even longer. Are you willing/able to make the financial and time commitment necessary to provide quality care for the
entire life of your companion rabbit(s)?
20. Do you already have a veterinarian who takes care of your companion animals? If so, please tell us who it is:
21. If you do not yet know a veterinarian who is experienced with rabbit medicine, would you be willing to allow us to refer you to
the best rabbit veterinarian(s) in the area, and to use their services?
Bunny Back Guarantee. If you adopt a rabbit from the H.A.R.E., Inc., will you agree that if, for any reason, your rabbit adoption
does not work out, that you will NOT turn the rabbit over to a friend, pet store, animal shelter or other person or organization, but that
you will instead contact your H.A.R.E., Inc. rescuer for assistance and work with H.A.R.E. until the rabbit can be taken in
again by H.A.R.E. (as a foster) or placed in a new home?
