What activity was undertaken as part of the project during 2012/13
This should be a description of the work undertaken as part of the project
during the financial year. If specific interventions were delivered, this section
should describe the work undertaken to deliver the interventions. If a post was
funded, this section should describe the main duties undertaken by the post
and how these were addressing the identified problem.
What outputs were delivered by the project
Outputs refer to the tasks that had to be done in order to achieve the
outcomes of the project (the final results of the intervention).
What problems were identified in delivering the objectives of the project
and how were these addressed?
Many reasons may arise or potentially postpone the delivery of projects.
Please indicate here which kind of issues arose – foreseen or otherwise, how
you have overcome them and by which actions. You should indicate also
which future actions are envisaged to try to resolve the problems if a solution

Were there any particular factors that assisted in the delivery of
objectives of the project?
Sometimes, there may be factors, planned or otherwise, which assist in the
delivery of the objectives of a project. If this was the case, please indicate
here what they were. This is particularly important in order to develop our
knowledge of best practice and what works since a project may also have
been a success due to local factors. We would seek to share these factors
with similar projects with other CSP’s across Suffolk.
